만성 정신장애인의 가족관계에 관한 질적 연구

초록

본 연구의 목적은 정신질환 발병 이전부터 발병 이후 수많은 어려움을 겪으며 회복해 가는 삶의 전 과정에서 만성 정신장애인이 경험하는 가족과의 관계를 고찰하는 것이다. 이를 위해 정신재활시설을 이용하는 7명의 참여자를 심층면접한 후, 일반적 질적 연구의 귀납적 주제분석 방법을 사용하여 분석하였다. 분석 결과, 9개 주제 즉, ‘소외와 고통을 준 병전 가족관계’, ‘병전 가족관계로 인한 욕구’, ‘발병 후 돌봄의 좌절’, ‘발병 후에도 모호한 가족의사소통’, ‘증상에 대한 가족의 무관심’, ‘증상에 대한 부적절한 대처’, ‘회복의 원동력’, ‘가족관계의 변화’, ‘회복의 결과’가 도출되었다. 참여자들은 발병 전부터 가족관계에 어려움이 있었고, 발병 이후에도 가족으로부터 지속적인 소외와 치료과정에서의 부적절한 대처를 경험하였다. 하지만 가족구성원 중 일부와 지지적이고 친밀한 관계를 맺은 경험은 참여자들에게 회복의 원동력으로 작용하여 전환점을 맞이하게 되고, 전반적인 가족관계와 가족의사소통에서 변화를 경험하였다. 참여자들은 가족으로부터는 존중․수용받는 경험을 하면서 자존감을 회복하고 삶에 대한 희망을 갖게 되었으며, 이는 다시 회복의 촉진이라는 선순환 과정으로 연결되었다. 이러한 연구결과를 근거로, 정신장애인과 가족과의 긍정적이고 지지적인 관계형성이 중요함을 강조함과 동시에 이를 유지하기 위한 개인, 기관 차원의 사항들을 제언하였다.

Abstract

The purpose of this study was to gain a deeper understanding of the family relationships of people with chronic mental disorders through the course of their disorders. In-depth interviews were conducted with seven individuals with chronic mental disorders who use daytime mental health rehabilitation centers in Gyeonggido, and data were analyzed using the inductive subject analysis method of generic qualitative research. As a result, nine topics were derived as such: premorbid relationships with alienation & pain, desires, disruption of care, ambiguousness, indifference, impropriety, drive for recovery, change, and result of recovery. Participants had difficulties in family before the onset, and they experienced alienation from their families continuously after the onset and also face dinappropriate coping during the treatment process. However, the experience of some of the family members' supportive relationship served as a driving force for the recovery, and they experienced changes in overall family relationships and communication. Participants recovered their self-esteem and had hopes for life while experiencing respect and acceptance from their family members, connected to a virtuous cycle process of promoting recovery.
