혈액투석 환자의 편의성 증진을 위한 의료서비스 개선안 연구

연구배경: 2017년 현재 국내 혈액투석 환자는 68,853명으로 인구 100만 명당 1331.9명에 이르며, 연간 약 5~8% 증가하고 있다. 혈액투석 환자의 증가로 인해 혈액투석 치료도 크게 발달하여 혈액투석 환자의 5년 생존율은 62.2%에 달했으며, 최근에는 단순한 생명유지 차원을 넘어서 지속적인 삶과 간호의 질적 만족도에 대한 관심이 높아지고 있다. 연구방법: 서비스디자인 방법을 이용하여 인공신장실 내 이해관계자들의 관계, 역할, 입장별 행동패턴, 요구사항과 의료서비스 실태를 분석하여 혈액투석 환자의 편의를 향상시키실 수 있는 의료서비스 개선안을 도출하였다. 연구결과: 혈액투석 환자들은 혈액투석과정, 시간 및 일정, 환자 상황(또는 상태), 복약/투약, 식단 및 건강관리 등에 대한 정보의 제공과 실시간 정보전달 및 공유가 가능한 쌍방향 소통 매체(또는 도구)의 지원에 대한 기대가 높은 것으로 나타났다. 결론: 연구의 결과는 혈액투석 환자들의 요구사항을 충족시킬 뿐만 아니라 원활한 소통과 효율적인 치료활동을 촉진하여 혈액투석 환자의 지속적인 건강관리 증진에 기여할 것으로 기대된다.
Abstract
Background: As of 2017, domestic hemodialysis patients are 68,853, implying the prevalence of 1331.9 per a million population, and they are increasing by approximately 5 ~ 8% per year. Owing to the increase of hemodialysis patients, the hemodialysis treatment has been significantly developed so that the 5-year survival rate of hemodialysis patients reached 62.2%. In recent years, there has been a growing interest in promoting continuous and everyday quality of life and nursing beyond simply maintaining life. Methods: The service design method was employed to analyze stakeholders’ relations, roles, behavioral patterns, requirements, and medical service status in artificial kidney room. Based on this results, a medical service improvement plan that can improve the convenience of hemodialysis patients was deducted. Result: Hemodialysis patients showed high expectations for 1) the timely provision of information on hemodialysis processes, time and schedule, patient situation (or status), medications, diet and health management, and 2) the support of interactive communication media (or tools) that can communicate and share information in real time. Conclusion: The results of this study are expected not only to meet hemodialysis patients’ needs, but also to facilitate unceasing communication and efficient treatment interventions, thereby contributing to the ongoing and everyday health promotion of hemodialysis patients.
