본 연구의 목적은 최근 급격하게 증가하고 있는 국민 의료비 지출의 큰 원인이 되고 있는 병원의 단기비용함수를 추정하고, 이를 이용하여 병원의 효율성을 확인하는데 있다. 본 연구는 2000년에서 2003년까지의 우리나라 57개 종합병원의 4개년도 패널자료를 이용하여 병원의 단기비용함수를 추정하고 효율성에 영향을 미치는 요인을 확인해보았다. 병원의 단기비용함수를 추정하기 위해 ‘translog multiproduct cost function’을 이용하여 고정효과모형으로 추정하였고, 규모의 경제를 확인하는 방법으로 ‘방사형 규모의 경제(ray scale economies)’를 이용하여 산출하였다. 단기적으로 병원의 비용과 유의적인 양의 상관관계를 갖는 변수들은 입원환자수, 외래환자수, 등록의사수, 의료기기고정자산, 자동차보험환자수로 밝혀졌다. 규모의 경제는 병원의 규모를 나타내는 고정투입물 중에서 유의적인 변수로 밝혀진 의료기기고정자산을 이용하여 산출하였다. 산출결과 우리나라의 종합병원은 규모의 경제 영역에서 운영되고 있었다. 이것은 단기적으로 최적설비 이상의 초과설비를 보유하고 있음을 의미하는 것이다.
Abstract
The purpose of this study is to estimate the hospital cost function and to check the efficiency of hospital investment based on the cost function estimation. For this purpose, this study uses a four-year panel data from 2000 to 2003 of 57 Korean general hospitals. For the short-run cost function estimation, we use the multi-product translog cost function and fixed effect model. For evaluating the scale economy of hospitals, ray scale economies were used. In the short run cost function estimation, number of inpatients, number of outpatients, number of admitting physicians, fixed assets of medical equipment, and number of car insurance related patients are found to be significantly increasing hospital costs. Calculation of ray scale of economies using short-run cost function estimation shows that Korean general hospitals have been operating within the area of scale economies, and it has been found that hospitals possess excess capacity rather than optimal capacity.
