선호추출법을 이용한 건강보험료 추가지불의사에 대한 일고찰: 폐암치료의 보장성 강화를 중심으로

본 연구는 이산선택실험의 방법을 이용하여 우리나라 건강보험 가입자를 대상으로 폐암의 치료에 대한 보장성을 강화하는 혜택을 누리기 위해 얼마의 건강보험료를 추가로 지불할 의사가 있는지를 추정하고 있다. 조건부 로짓과 혼합 로짓 모형을 이용하여 1,000명의 응답자를 대상으로 실시한 설문의 자료를 분석하였으며, 사회경제적 요소를 포함한 교호항을 포함하여 이들이 추가 지불의사금액에 어떠한 영향을 미치는가를 고찰하였다. 그 결과 폐암치료의 평균 연간 치료비용에 대한 추가적인 건강보험료 지불의사금액은 가구당 3,256원에서 5,638원에 달하는 것으로 추정되었다. 이는 기존의 물가상승 및 급여항목의 변화에 따라 인상폭이 책정되었던 건보료 결정방식에 대하여 가입자의 자발적인 지불의사금액 의사 표출을 바탕으로 건보료를 조정하는 방식에 대한 기초연구로서 그 의의가 있다.
Abstract
This study, using a preference elicitation method, tries to estimate the marginal willingness to pay for national health insurance premium of the insured if the insurance coverage is expanded for the medical treatment costs of the patients with lung cancer. Based on the survey with 1,000 representatives currently insured in national health insurance service, the study adopts various econometric methods to estimate the amounts that range KRW 3,256-5,638 per household on a monthly basis. Adding several variables of socioeconomic status show minor discrepancies compared with the results of basic analysis with attributes related with lung cancer.
