본 연구는 알츠하이머형 치매환자들에게 무용프로그램의 리더역할을 제공할 경우에 나타나는 자기표현력의 변화를 검증하는데 목적이 있다. 특히, 알츠하이머형 치매환자의 증세분류에 따라 혈액형치매, 조증치매, 그리고 울증치매가 나타내는 자기표현력을 분석하였다. 적용된 무용프로그램은 Elizabeth Rosen이 시행한 ‘요구와 역할모델’이었다. 대상자는 55명의 알츠하이머형 치매환자였으며, 실험집단(34명)과 비교집단(21명)이 포함되었다. 자기표현력의 세부영역은 신체표현, 대화자세, 그리고 언어표현이었으며, 실험집단의 사후검사 결과는 유의미한 향상을 나타냈다. 그러나 비교집단의 자기표현력 세부항목들은 통계적으로 유의미한 향상을 나타내지 않았다. 실험집단의 대상자들을 장애유형별로 분석한 결과는 다음과 같다. 첫 번째, 신체표현의 경우, 조증치매환자가 가장 높은 증가를 나타냈으며, 우울증을 동반한 대상자, 그리고 혈관성치매를 가진 대상자의 순서로 증가하였다. 조증치매와 울증치매의 신체표현력은 통계적으로 유의미한 증가를 나타냈다. 두 번째, 대화자세는 우울증을 동반한 치매환자들이 통계적으로 유의미한 점수 증가를 나타냈으나 혈관성치매를 가진 대상자와 조증치매를 가진 대상자의 점수증가는 통계적으로 유의미하지 않았다. 이러한 결과는 울증치매환자들에게 리더역할을 주었을 때, 주어진 상황을 차분하게 서술하는 경향이 나타났기 때문이며, 조증치매환자의 조급함과 과잉행동으로 인한 점이 작용한 것으로 예측된다. 세 번째, 언어표현은 조증치매환자와 울증치매환자들에게 유의미한 점수증가가 나타났다. 그러나 혈관성치매환자의 언어표현은 유의미한 향상을 나타내지 않았다. 본 실험의 결과는 조증치매환자에게 ‘집단의 리더’라는 자극적인 활동을 제공할 때, 평소보다 명확한 자기표현을 나타냈으며, 울증치매환자도 집단의 리더임무를 성공적으로 수행하고 명확한 의사표현을 수행한다는 점이 발견되었다. 이러한 점을 고려할 때, 경조증을 나타내거나 의료적 치료중인 울증치매환자들에게 적절한 역할을 부여하고 계획과 진행, 그리고 결과를 평가할 수 있는 기회를 제공하는 것은 인지결손으로 인한 부적절한 자기표현을 개선할 수 있는 방안이 될 것이다.
Abstract
This study investigated the specific effects of a dance intervention on the increase of self-expression skills such as gestures, attitudes, and verbal selections in 55 dementias of Alzheimer patients with additional diagnosis of depression, manic, or vascular. Experiment group includes 34 Patients participated in a dance group performing with assignments of leader role, meanwhile a control group(n=21) with free assignments participated in a line dance. Results suggest that patients in the experimental group profited most from the intervention. They showed significantly increase of self-expression skills than participants in the control group. Results are as follows: firstly, levels of desirable gestures during conversation for the samples with manic are significantly increased. Secondly, levels of desirable conversation attitudes of the samples with depression are also significantly increased. Thirdly, levels of verbal skills of the samples with manic are significantly increased in the experimental group. The results indicates that negative gestures and attitudes in a conversation of the dementias of Alzheimer patients with depression, manic, or vascular were decreased by using leader role-assignment.
