단계기반 접근에 근거한 복합외상후스트레스 증상완화 프로그램의 개입효과 검증에 관한 예비연구: 단일 사례 연구

초록

이 연구의 목적은 복합외상후스트레스 증상을 가진 내담자를 대상으로 증상 완화 프로그램을 실시한 후 그 효과를 검증하는데 있다. 이를 위해 복합외상후스트레스 증상 완화를 위한 프로그램 개발 및 실시하였으며, 프로그램으로 인한 심층적 변화를 살펴보기 위하여 단일사례 연구를 실시하였다. 이러한 연구목적을 달성하기 위해 인지행동치료를 근거로 안정화 기법, 외상 기억 처리, 통합의 과정을 중심으로 매주 90분씩 4세션에 걸쳐 진행하였다. 프로그램 효과성을 검증하기 위하여 사건충격정도, 아테네불면척도, 심장박동변이를 사전-사후-추후 검사하였다. 또한 상담과정에 대한 녹취록과 비언어적 관찰과정을 자료 수집하여 사례분석을 실시하였다. 분석결과 첫째, 프로그램에 참여한 내담자의 사건충격정도, 수면의 질, 심장박동변이는 유의미하게 변화하였다. 둘째, 상향식 접근법과 하향식 접근법은 복합외상후스트레스 증상을 완화시키는 것으로 나타났다. 이 연구는 복합외상후스트레스 증상 완화를 위해서 단계기반 접근을 상향식․하향식 양방향 접근법으로 프로그램에 적용하여 그 효과성을 살펴보았다는 것에 의의가 있다. 연구결과를 바탕으로 복합외상후스트레스 증상 완화 프로그램의 방향성과 정책 제언에 대한 함의를 제시하였다.

Abstract

This study aims to implement a therapy program with a client who have the symptoms of the complex post-traumatic stress disorder (CPTSD) and evaluate its effects. For this purpose, a program for the relief of the symptoms of the CPTSD was developed and implemented, and a single-case study was conducted to examine the deep change caused by the developed program. In order to achieve the study purpose, based on cognitive behavioral therapy, focusing on stabilization treatments, trauma memory processing, and integration process were conducted in four sessions of 90 min each week. To evaluate the effectiveness of the program, pre-, post-, and follow-up tests were conduced with Impact of Event Scale-revised Korean version(IES-R-K), Athens Insomnia Scale (AIS), and heart rate variability (HRV), and the recorded tapes of the counseling process and data on the non-verbal observation process were collected and case analysis was conducted. As a result of the analysis, first, it was found that the IES-R-K, AIS, and HRV of the client who participated in this program were changed significantly. Second, it turned out that the bottom-up approach and the top-down approach alleviated the symptoms of the CPTSD. This study has significance in that it investigated the effectiveness of the program that applied the bilateral therapy based on phase-based approach for the relief of the symptoms of the CPTSD. Based on the result of this study, directions for the relief of the symptoms of the CPTSD and implications for the policy proposal were proposed.
