이 연구의 목적은 정신분열장애의 특징을 살펴보고, 정신분열장애의 이해와 개입에 대한 맨체스터 모형을 분석하는 것이다. 그리고 이 분석을 근거로, 정신분열장애에 대한 기독교상담의 원리를 구성하는 것이다. 정신분열장애의 전형적 진단 기준은 망상, 환각, 와해된 언어, 기괴한 행동, 그리고 음성증상들이다. 이외에도 신경인지기능의 결핍, 심리적 외상, 불안, 우울, 자해, 자살, 중독, 학대, 폭력가해 등과 같은 증상도 동반될 수 있다. 그리고 평생 지속되는 장애로서, 전구기, 급성기, 부분관해기, 완전 관해기, 그리고 재발 전구기의 단계로 진행되는 경향이 있다. 정신분열장애에 대한 맨체스터 모형은 인지행동적 접근이다. 이것은 대처를 통한 회복을 강조한다. 그리고 정신분열장애를 내적 요인과 외적 요인의 역동적 상호작용의 결과로 이해한다. 그리고 증상이 재발하는 과정을 경험-신념-행동-확증의 순환과정으로 분석한다. 주로 증상의 경험에 대한 주관적 평가와 반응행동에 개입해서 증상의 재발을 감소시킨다. 개입의 목표는 다양하다. 정신분열장애 내담자의 문제가 다양하고 복합적이기 때문이다. 심리적 문제, 심리사회적 문제, 그리고 사회적 문제를 포함한다. 그리고 개입의 방법은 상호존중, 강정중심, 그리고 발달적 관점으로 이루어지는 심리교육이다. 특히, 속도, 범위, 체계, 유연성, 확장가능성에 손상을 갖고 있는 인지과정을 고려한다. 맨체스터 모형을 근거로 정신분열장애에 대한 기독교상담의 원리를 5개로 제시한다. 첫째, 목표는 정신분열장애와 함께 살아가는 방법의 학습이다. 둘째, 개입은 타 기관이나 전문가와 연계해서 진행한다. 셋째, 상담의 과정과 내용은 정신분열장애의 인지과정에 적합하게 구성한다. 특히, 내담자가 소속한 기독교공동체의 생활을 중심으로 구성하면 효과가 더 크다. 넷째, 동기의 증진과 대인관계의 발달에 초점을 둔다. 다섯째, 상호존중, 강점중심, 그리고 장기적 관점을 유지한다.
Abstract
The aim of this article lay in constructing some principles for Christian counseling with the schizophrenia based on Tarrier’s Machester model. Its understanding of schizophrenia, and interventive aims, methods and skills were analyzed, after symptoms and associated features in schizophrenia were reviewed. Key features of schizophrenia included delusions, hallucinations, disorganized speech, grossly disorganized or abnormal motor behavior, and negative symptoms. Its associated features included neurocognitive deficits, psychological trauma, anxiety, depression, self-harm, suicide, alcohol or substance addiction, and violence victims and perpetrators. Frequently it was composed of such five phases as prodromal, acute episode, partial remission, full remission, and relapse prodromal. The model had many similarities to other models in the cognitive behavioral approach. Its basic tenet was the recovery through coping model. It posited that coping with the potentially life-long disorder facilitate the process of recovery. It assumed that experience of symptoms resulted from a dynamic interaction between internal factors, biological and psychological, and external factors, environmental stress. And it analyzed the process interaction in that appraisal and response behaviors took a significant role in producing the symptoms. Its therapeutic aims were specific and multidimensional because the symptoms and difficulties in schizophrenia were multidimensional and individually differentiated. They included psychological, psychosocial, and social difficulties. Its core method included psychoeducation facilitating learning with the collaborative, strength-based, and developmental perspective. It took the slow, narrow, disruptive, and inflexible cognitive process into consideration. Five principles in Christian counseling were constructed based on the model. First, its aim was to cope with the symptoms of the disorder. Second, it took a team approach in which a client was served by many professionals such as psychiatrists, social workers, and so on. Third, its process and programs were constructed and modified according to the client’s slow, disruptive, and inflexible cognitive process. Fourth, it focused on enhancement of the client’s motivation and development of interpersonal relationships. Its counseling relationship contributed to the development. Fifth, it emphasized on the collaborative and strength-based perspective. The perspective could facilitate changing many aspects of life, enhancing hopes and aspirations, and reducing the associated features.
