애착 관점에서의 조현병 이해와 목회상담적 함의

조현병은 유전적 요인과 신체적 요인에 대해서는 약물치료를 중심으로 발전되어져 왔으며 심리치료에서는 인지 행동적인 치료연구가 있어왔다. 본 연구는 조현병의 심리 정서적 요인의 하나인 애착의 관점에서 조현병의 발병요인들과 치료 방법들을 논의하고 목회상담적 적용을 고찰하고자 하였다. 애착 관련 임상적 연구들의 결과들과 청년 조현병 내담자의 사례들은 불안정 애착과 조현병과의 연관성의 증거들을 보여주고 있다. 안정애착의 긍정적 대응전략을 가진 내담자들은 자신과 타인에 대한 신뢰가 있기에 위기상황에서도 높은 심리적 안정감과 적응력이 있다. 하지만 불안정애착 유형은 낮은 자존감과 타인에 대한 낮은 신뢰로서 정신질환 발병율이 높게 나타난다. 조현병 환자의 삼분의 이 이상이 불안정 애착 유형으로 분류되며 그들의 부정적인 대응전략들은 낮은 치료 참여도, 치료적 동맹 형성의 어려움, 낮은 회복 가능성에 연관성이 있다. 조현병 증상으로 말미암아 많은 조현병 환자들은 정상적인 사회생활과 기능을 할 수 없고 인지적 정서적 기능의 손상이 심해지는 경우도 있지만 안정애착경험이 있는 조현병 환자들은 적극적인 치료 참여도와 높은 치료적 동맹 긍정적인 대응전략을 통해서 회복 수준이 높게 나타나고 있다. 애착 중심적 치료는 조현병 내담자들에게 치료자와 안정애착 경험을 갖게 하면서 치료적 동맹을 강화시키며 그 치료기법들은 내담자의 내적작동모델을 탐색하면서 정신화 능력을 강화시키고 인지 행동 기능과 사회성 기술들을 향상시킨다. 본 연구는 애착중심 치료전략들이 조현병 내담자들과 가족들을 위한 통합적 목회상담을 위해 고려되어야 할 요소로 보고 적용점들을 제시하였다.
Abstract
While mental health studies on schizophrenia have developed pharmacotherapy based on biological and physical factors, psychotherapies have dealt with behavioral and cognitive treatments. This study examined causes and treatment strategies of schizophrenia from an attachment perspective and discussed pastoral applications. Studies of attachment theory show that a child’s secure attachment with parents leads to emotional security and psychological well-being. This study provided evidence of the connection between insecure attachment and schizophrenia through findings of empirical studies and clinical cases of young adults with schizophrenia. Clients with positive coping strategies tend to have confidence in themselves and others and a high level of psychological coping and adjustment ability in the midst of crisis. However, individuals with insecure attachment styles present low self-esteem and low trust level on others and report a high incidence of mental disorders. Two-thirds of schizophrenia patients are classified as insecurely attached, and their negative coping strategies are strongly related to low commitment to treatment, difficulty in building a therapeutic alliance, and poor treatment outcomes. Many schizophrenia patients lose normal social functioning skills and cognitive and affective functions. However, the patients with secure attachment experiences report a higher recovery level through a strong commitment to treatment and therapeutic alliance. Therapies focused on attachment help clients with schizophrenia to have a secure attachment experience and improve their therapeutic alliance. The treatment interventions based on attachment explore the client’s internal working models and strengthen the mentalization and improve his or her cognitive and behavioral functions and social skills. Finally, this study provided suggestions for integrative pastoral applications based on the treatment strategies focused on attachment.
