암 환자에 대한 정신종양학의 완화치료와 기독교상담의 영적 지지

이 연구는 암 환자에 대한 정신종양학의 완화치료와 기독교상담의 영적 지지를 활용하여 암 증상 가운데 통증과 섬망을 완화하는 것에 대한 것이다. 암은 전 세계적으로 꾸준히 증가하는 질병으로서 우리나라의 경우, 사망 순위 1위를 차지하는 질병이 되었다. 암은 명백하게 신체적 질병이지만, 대부분의 사람들은 암에 노출되면, 심리정서적, 영적 문제도 함께 발생한다. 암은 발생 부위나 암종에 따라 의학 전문영역에서 다루게 되지만, 오심, 구토, 피로, 통증, 수면장애, 악몽, 식욕부진, 섬망 등의 신체적 증상과 불편감을 비롯한 우울, 불안 및 디스트레스 등의 심리정서적 문제는 정신종양학(psycho-oncology)에서 담당한다. 정신종양학에서는 암 환자에게서 나타나는 신체적, 심리정서적 증상제거 및 감소, 완화를 통해 암 환자의 삶의 질을 향상해 주는 것이 학문적 목표이며 이를 ‘완화치료’(palliative therapy)라 한다. 암 환자에게서 나타나는 증상은 매우 다양하지만 가장 심각한 주요 증상은 통증(pain)과 섬망(delirium)이다. 이것은 정신종양학의 완화치료적 접근으로만 할 수 있는 것이 아니고, 기독교상담의 영적 지지가 완화치료에 커다란 영향을 줄 수 있는 방편임을 이 연구에서 제시하였다. 기독교신자 암 환자에 대한 증상완화를 위해 영적 지지를 하는 것은 기본이며, 많은 암 환자들이 영적 지지를 받고자 희망하기 때문에 영적 지지를 확립하여 제공해야 한다. 암 환자에 대한 영적 개입에 있어서 기독교상담자의 역할이 중요하다는 판단 아래 이 연구에서는 1) 암 환자에게서 발생하는 다양한 심리정서적 측면을 다루었고, 2) 정신종양학의 완화치료에 대한 이해를 다루었으며, 3) 완화치료에서 방편으로서 활용할 수 있는 기독교상담의 영적 지지방법에 대해서 살펴보았다. 영적 권위를 가진 기독교상담자, 병원 원목 및 목회자는 기독교인 암 환자에게 영적 돌봄을 제공하는 지지자로서 가장 적합하고, 효과적이다. 기독교인 암 환자뿐만 아니라 비기독교인 암 환자들도 기도를 포함하여 다양한 영적 자원을 통한 도움을 필요로 하고 요청하고 있다. 기독교상담자는 말씀을 통한 인지적 재구성과 기도를 통한 영적 지지를 통해 암 환자의 고통과 섬망을 다루어야 하며, 효율적으로 다룰 수 있는 존재이다. 즉, 성경은 인지적 재구성을 통해 암 환자가 겪는 현재의 상황과 의미를 영적으로 재해석하여 극복하는데 영향을 미친다. 이 연구에서는 기독교인 암 환자가 통증을 호소할 때 영적 차원에서 지지적 접근으로써 암 통증을 경감시킬 수 있다는 것을 살펴보았다. 통증과 아울러, 섬망에 대한 완화도 재론의 여지없이 정신종양학의 주요분야로서 완화치료의 역할이라 할 수 있다. 섬망은 대개 암이 급격하게 진행되는 상황에서 발생하여 암 환자의 가족과 의료진을 당황하게 하기 때문에 완화치료적 접근이 급히 요구되는 상태이다. 암 환자의 섬망 진단은 DSM-5에 의존하고 있으며, 치료는 정신종양학의 완화치료적 접근을 하는데 이에 기독교상담적 접근으로서 영적 지지는 암 환자의 섬망을 감소하며, 심리정서적, 영적 안정을 갖도록 하는데 효율적인 방편이라 할 수 있다.
Abstract
This article is to clarify on a combination between spiritual support of Christian counseling and palliative therapy of psycho-oncology for Christian cancer patients. Cancer patiences are proliferating worldwide. The researcher dealt with pain and delirium of cancer patients as a core theme of psycho-oncology. The researcher also dealt with Christian counselors’ role for spiritual intervention to improve quality of life through palliative support for cancer patiences. First of all, this researcher studied 1) various psychological aspects of cancer patients, 2) palliative therapy of psycho-oncology and 3) approach of Christian counseling for palliative care. It is effective for Christian counselors, hospital chaplains and pastoral ministers who have spiritual authority to provide spiritual care for cancer patients. Christian cancer patients as well as non-Christian cancer patients are requiring spiritual support including prayer by spiritual resources. The main symptoms of cancer patients are pain and delirium. Christian counselors should deal with the cancer patients’s pain and delirium by cognitive reframing, spiritual support through the Bible and prayer. That is, the Scripture has an definite effect on changing the situation and meaning of cancer patients through cognitive reframing spiritually. Prayer also has obvious effect on alleviating the pain and delirium cancer patients.
