남성 알코올 의존 환자의 삶의 질과 정신-사회-영적 특성과의 관계

본 연구는 알코올 의존 환자의 삶의 질과 이에 영향을 미치는 정신, 사회 그리고 영적 특성과의 관계를 알아보고, 삶의 질을 측정하는 도구의 특성도 비교하고자 하였다. WHOQOL 삶의 질 척도와 다른 변인들과 보인 관련성은 대체로 교육수준이 높을 수록, 월수입이 많을수록, 음주 정 도가 심하지 않고, 음주의 부정적인 결과가 덜 심각할수록, 불안과 우울이 심하지 않을수록, 사회적 지지 수준이 높을수록 그리고 종교적 믿음으로 표현되는 행동이 빈번할수록 높은 삶의 질 수준을 보였다. SBQOL 삶의 질 척도와 다 른 변인들과 보인 관련성은 수입이 많을수록, 음주의 부정적인 결과가 덜 심각할수록, 불안과 우울이 심하지 않을수록 높은 삶의 질 수준을 보였다. 또한 불안과 우울 등 심리적 요인이 알코올 의존 환자의 삶의 질에 가장 큰 영향을 주었다. 이는 알코올 의존의 회복에 있어서 심리적 요인에 대한 적절한 도움을 주는 것이 매우 중요함을 시사한다. 또한 알코올 의존 환자의 삶의 질을 측정하는 보다 높은 신뢰도와 타당도를 지닌 평가도구에 대한 후속 연구가 필요하다고 생각된다.

Abstract

Objectives：This study was designed to evaluate the relationship between quality of life (QOL) and psycho-socio-spiritual characteristics in male patients with alcohol dependence. Methods：The sample consisted of 109 men with alcohol dependence defined by DSM-IV criteria. We assessed QOL by the WHO QOL assessment instrument-BREF (WHOQOL) and SmithKline Beecham QOL (SBQOL). Sociodemographic and alcohol related data were collected, and 7 questionnaires were administered: MAST, BDI, STAI, Drinker Inventory of Consequences (DrInC), Scale of Social Support (SSS), Religious Beliefs and Behaviors (RBB) and Spiritual Well-being Scale (SWBS). The correlations between each QOL score and other variables were examined, and stepwise multiple regression analysis was performed. Results：The WHOQOL score positively correlated with education level, SSS (support) and RBB and negatively correlated with MAST, DrInC, BDI, STAI (trait) and SSS (conflict) scores. In stepwise regression analysis, the scores on the STAI and BDI contributed to the score on the WHOQOL. The SBQOL score correlated with the income level, and negatively correlated with BDI and STAI score. STAI score was a weak predictor of SBQOL score. Conclusion：The significant predictors of QOL in patients with alcohol dependence were psychological factors such as anxiety and depression. (J Korean Neuropsychiatr Assoc 2006;45(5):459-467)
