정신종양학의 역사와 현황

History and Current Status of Psycho-Oncology

초록

짧은 역사에도 불구하고, 정신종양학은 괄목할 만한 진보를 했다. 암의 ‘인간적’ 측면에 대해 축적된 상당한 수준의 지식은 암 환자의 치료에 영향을 주었다. 암환자에 대한 심리사회적 서비스는 환자의 삶의 질 향상 효과와 더불어 전반적 의료비용을 절감하는 효과도 있다47,48). Simpson 등49)의 유방암 환자를 대상으로 한 전향적 연구에서는 심리사회적 중재가 삶의 질 향상 효과뿐 아니라 치료그룹에서 23.5%의 치료 비용 감소 효과를 가져온 것으로 보고하였다. 암환자의 디스트레스를 여섯 번째 활력 징후로서 선별하고 관리할 것을 권고한 캐나다의 CSCC는 그렇게 하지 않았을 경우에 더 심각한 결과와 비용이 발생할 수 있다는 것을 인정하였다.33) 그러나 아직 한국의 암 진료 현장에서는 이러한 심리사회적 서비스가 암환자 치료의 일부로 통합되고 있지 못하다. 이를 위해서는 우선 암환자와 가족, 종양의료진, 대중 사이에서 암의 심리사회적 측면에 대한 이해와 관리 필요성이 인식되어야 한다. 암환자의 고유한 심리사회적 문제들에 경험이 많은 정신종양학자(Psycho-oncologist)의 양성도 시급하다. 암 센터 내에 정신종양학이 암환자와 종양의료팀에게 암환자의 통합적 치료의 일환으로 인식되는 것도 중요하다. 정신종양학적 지식들은 대부분 서구 선진국의 경험에서 도출된 것이다. 따라서 인종, 문화, 사회경제적 상황 등을 초월하여 적용되는 보편적 측면이 있는 반면, 그렇지 않은 부분도 있을 수 있다. 그러므로 이러한 서구 선진국에서 발전한 정신종양학의 적용은 한국 암환자와 암 진료 상황을 고려하여 신중하게 이루어져야 할 것이다. 암 발병률이 보여 주듯 암은 대다수의 한국인들에게 직접적 혹은 간접적으로 간과할 수 없는 중요한 현실이다. 암이 환자와 가족 및 사회에 미치는 고통은 심각하다. 정신종양학이 종양학 내에 확고한 분과로서 자리매김하는 것은 전인적인 의료를 지향하는 정신의학의 시각에서 볼 때 중요한 의미를 가진다.

Abstract

Cancer statistics shows a high risk and increasing incidence of cancer among Koreans. Considerable burdens related to cancer and its treatment causes significant psychosocial distress among cancer patients and their family. Due to early detection and progress in cancer treatments, cancer is becoming a chronic illness and the quality of life and long-term impact of cancer is receiving increasing attention. Psycho-oncology is a subspecialty of oncology that addresses a psychosocial dimension of cancer throughout ‘the continuum of cancer care’, viz. from prevention and early detection to palliative and end-of-life care. This paper reviews the development of psycho-oncology and its current status in international and national context so as to promote efforts toward an integrative cancer care in Korea.
