아동학대 생존자의 외상 후 성장을 위한 인지행동치료 프로그램의 개발 및 효과

초록

본 연구의 목적은 아동학대 생존자의 외상 후 성장을 위한 인지행동치료 프로그램을 개발, 실시하고 그 효과를 알아보는 것이다. 이를 위해 아동기 학대를 경험한 성인을 대상으로 아동기 외상 질문지 및 외상 후 진단 척도를 실시 후 선별된 24명이 실험집단, 비교집단, 통제집단에 각각 8명씩 무선 배정되었다. 실험집단을 대상으로 인지행동치료 프로그램을, 비교집단을 대상으로 이야기치료 프로그램을 각각 주 1회 매 회기 90분씩 총 10회를 진행하였으며, 통제집단에게는 아무런 처치도 실시하지 않았다. 본 연구의 주요 결과는 다음과 같다. 첫째, 집단 간 사후검사 비교 시 실험집단은 비교집단과 통제집단에 비해 죄책감, 우울, 거절불안, 과순응성이 낮은 것으로 나타났다. 또한 실험집단과 비교집단은 통제집단에 비해 자부심이 높은 반면, 비주장성과 친밀감에 대한 두려움은 낮았다. 한편 수치심의 경우, 실험집단, 비교집단, 통제집단의 순으로 낮게 나타났다. 둘째, 프로그램 종료 3개월 후 집단 간 추후검사 비교 시 실험집단은 비교집단에 비해 자부심은 높은 반면, 그 외 모든 변인들은 낮은 것으로 나타났다. 셋째, 프로그램 실시 후 실험집단의 자기지각과 대인지각 관련 부적응적 사고 및 신념은 적응적인 변화를 나타냈다.

Abstract

This study’s purpose is to develop and apply a cognitive-behavioral therapy program for post-trauma growth of child abuse survivors and to further evaluate their effectiveness. For this purpose, 24 children selected after the childhood trauma questionnaire and post-trauma diagnosis scale were randomly assigned to the experiment group, the comparative group and the control group, respectively (n = 8). The cognitive-behavioral therapy program for the experimental group and the narrative therapy program for the comparative group were performed 10 times for 90 minutes once a week for each group, and no treatment was provided for the control group. The major results of this study are as follows. First, the experimental group showed lower guilt, depression, rejection anxiety, and over-compliance than the comparative group and the control group in the post-test comparison. Furthermore, the experimental group and the comparative group had higher self-esteem than that of the control group, while the fear of non-assertiveness and intimacy was lower. Shame, also, was shown as low in the order of experimental group, comparison group, and control group. Second, in the post-test comparison between the groups, after three months of the program, the experimental group showed higher self-esteem than the comparative group, while showing all other variables as lower. Third, after the program, the experimental group showed adaptive changes in self-perception as well as maladaptive thinking and belief related to the interpersonal perception.
