ADHD 아동의 WCST 수행 저하와 실행기능 및 인성 특성과의 관계 : WCST 수행 정상 ADHD 및 우울 집단과의 비교

초록

본 연구는 위스콘신카드분류검사(Wisconsin Card Sorting Test; 이하 WCST)수행이 ADHD 아동의 핵심적 증상인 과잉행동과 충동성, 부주의와 더불어 어떠한 요인의 영향을 받는지 알아보고자 하였다. 그리고 WCST가 ADHD 아동과 유사한 전두엽 기능 문제를 드러내는 우울집단에서의 증상을 차별적으로 드러낼 수 있는지 확인해 보고자 하였다. 이런 목적을 위해, 본 연구에서는 WCST수행을 통해 두 집단으로 분류하여 인지, 정서, 행동적 측면을 비교하였으며, 다음으로 WCST수행 정상 아동과 우울집단 아동을 비교하였고, 마지막으로 ADHD 아동과 우울집단 아동을 비교하였다. 그 결과. WCST 완성범주 수에 따른 집단 비교에서는 WCST 하위영역점수가 유의한 차이가 있었고, 한국판 아동인성검사(KPI-C)의 자아탄력성, 언어발달, 우울, 사회문제 척도에서 차이가 있었다. WCST 수행 정상 집단과 우울집단 간에는 아동인성검사의 신체화, 우울척도에서 차이가 있었고, 선로 잇기 B형 검사 반응시간, 스트룹 검사 색 단어 색깔 말하기 반응시간, WCST 완성범주수에서 유의한 차이가 있었다. 마지막으로 ADHD 아동과 우울장애 집단 간에는 선로 잇기 B형 검사 반응시간, 오류 수, 스트룹 검사 색깔글자 색깔 말하기 반응시간에서 유의한 차이가 있었으며, 아동인성검사의 신체화와 과잉행동 척도에서 유의한 차이가 있었다.

Abstract

This study investigated if Wisconsin Card Sorting Test(WCST) performance was influenced by which factor with core symptoms of ADHD children such as hyperactivity, impulsivity, and inattention. And, to identify the performance of WCST, and compared to difference for cognitive, affective, and behavioral aspects. Also, this study compared children group with normal performance of WCST, and with depression disorder. and do comparison ADHD and depression children group. In result, there were significant difference subscores of WCST, and subscale of K-PIC such as the ego resilience, verbal development, depression, social problem in comparison according to the completed category of WCST. There were significant difference at the somatization and depression scale of K-PIC, response time of Trail making B, Color naming of color word of Stroop, completed category of WCST in comparison nomal performance of WCST group between depression group, Finally, there were significant difference in comparison ADHD and depression group for response time and number of error of Trail making B, response time of Color naming of color word of Stroop, completed category of WCST.
