Abstract
We consider an economic model in which the medical treatment level and the health care insurance contracts are determined where the treatment level is selected by a doctor. Three cases are considered: when the doctor is benevolent, when the doctor maximizes the treatment efficiency, and when the doctor is selfish. A partial hedge is obtained when the doctor is benevolent, while a full hedge is obtained in other two cases. The full hedge may imply over-insurance if the treatment cannot fully recover the health level before getting sick. The treatment level is higher when the doctor is benevolent than when the doctor maximizes the treatment efficiency. However, the treatment level under the selfish doctor can be higher or lower than that in other cases. We discuss the possibility that moral hazard is overemphasized in the traditional health economics literature.
