노인성 질병에 대한 민영 보험의 수요추정과 보험수리 모형의 설계에 관한 연구

최근 노인인구의 비중이 지속적으로 증가하면서, 개인의 노후 대비와 노인복지가 중요한 문제로 부각되고 있다. 이와 관련하여 노년기의 질병 및 이에 따른 비용 지출은 개인의 재무적인 상황은 물론 삶의 질에 영향을 미치게 되는데, 특히 치매 등 장기 간병이 필요한 질병을 얻게 되는 경우에는 그 영향이 개인이 감당할 수 없을 정도로 커질 수 있다. 이러한 문제를 해결하기 위해 국내에서는 사회보험의 형태로 노인장기요양보험이 2008년부터 4년 여간 운영되어 오고 있다. 그런데, 노인장기요양보험의 재정적 한계로 인하여, 장기간병이 필요한 질병을 가지고 있는 모든 사람이 해당 제도의 혜택을 받지 못하고 있는 실정이다. 따라서 본 연구에서는 2008년 보건복지부에서 시행한 치매 노인 유병률 조사 및 노인장기요양보험의 실적자료를 이용하여 적절한 수리적 모형을 설계하여, 이를 바탕으로 향후 치매 유병자 수를 추산하고, 노인장기요양보험의 인정자 수 추이와 비교 분석하여 민영보험의 역할에 대하여 논의하였다.
Abstract
Personal plans for later years and social welfare for the elderly become an important issue as the number of elderly population is getting increased. Suffering from a disease and related cost affect financial status and quality of life of elderly people. Especially, a disease that requires long term care such as dementia will have impact that an elderly person is not able to manage. In order to resolve the problem, public long term care insurance system has been operating around 4 years. However, due to its financial limit, not all person who needs long term care service is eligible for public long term care service. This study explored designing appropriate model based on the Survey of prevalence of dementia in elderly population performed by Ministry of Health and Welfare in Korea and experience data obtained from public long term care insurance of Korea so as to estimate the number of people who suffer from dementia and the number of people who are eligible for public long term care insurance. Finally, the role of private insurance supporting public long term care insurance is discussed.
