수발정책의 비공식적 영역지원에 대한 주요 국가별 비교연구

인구고령화로 인해 장기요양보호의 수요가 증가하여 수발을 사회와 공동체가 함께 부담하는 소위 ‘노인장기요양보험' 제도가 시행되면서 공식적 영역의 수발서비스 제공 체계가 시작되었다. 그러나 공식적 영역에서 제공되는 서비스가 욕구에 비해 턱 없이 부족하여 비공식 영역이 여전히 중요한 역할을 하고 있다. 더욱이 노인장기요양보험은 재가급여를 원칙으로 하고 있어 비공식 영역이 재가보호에 있어서 필수적임에도 그에 대한 적절한 지원방안이나 정책들이 마련되어 있지 않은 것이 현실이다. 이에 본 연구에서는 비공식영역의 정책이 복지국가 유형에 따라 어떻게 다르게 나타나는지 살펴보고 우리에게 알맞은 정책방향을 제시해 보았다. 영국과 독일을 분석한 결과 첫째, 복지국가 유형에 따라 비공식적 영역의 정책 차이가 있었다. 자산조사 모델은 비공식 영역의 자격제한과 사후관리를 통해 엄격한 감독 하에 급여를 제공하는 반면 북유럽 보충주의 모델은 급여만을 제공하고 이후의 사용에 대해서는 국가가 관여하지 않는 것으로 되어 있다. 둘째, 사회적 관점에 따라 여성주의 국가의 경우 여성 문제를 해결하기 위하여 비공식 영역을 공식화해 여성 노동력의 경제적 가치를 인정하고 있다. 셋째, 비공식 영역 제공자의 욕구에 관심을 둠으로써 서비스 제공에 따른 비용을 줄일 수 있는 대책들을 보여주고 있다. 따라서 독일과 영국의 연구 결과에 따른 분석은 비공식 영역에서 매우 제한적이고 초기 논의단계에 있는 우리에게 보다 효율적이고 유연한 정책적 지원체계를 마련하는데 시사점을 보여줄 것으로 판단된다.
Abstract
With the enforcement of the long term care Insurance, which the government and the public pay together for care-giving of the elderly, official care-giving services in Korea was enacted. Nevertheless, because the official services are insufficient in comparison with the increasing needs for long term care, unofficial care service is still considered as a important portion in elderly care. Moreover, the long term care Insurance of Korea is based on the principle of ‘home-based service’ that informal sector is necessary in caring for the elderly, yet the support system involving informal sector is not firmly established. In this background, this study aims to compare policies for informal sector in care services for the elderly by types of welfare states, in order to provide proper future policy suggestions for the home-based caring for elderly in Korea. The results of this study can be summarized as followings; First, there are significant differences in policies for unofficial sector among the different types of welfare states. The countries being classified into the ‘means-test’ or sometimes called ‘Beveridge-model’, provides official services under the scheme of regimented supervision on use of benefit on unofficial service. On the contrary, the northern European countries, being classified into the northern European subsidiarity model, also provide benefit but do not restrict its use for unofficial care. Second, there were differences in policies for unofficial sector by societal perspective. Feminist country recognizes financial value of female labor forces and arranges formal support system for the unofficial care-givers, while the familistic country takes a complementary role on care giving and does not provide benefits for unofficial care giving. Third, there can be policy alternatives that reduce care service expenditure by focusing on demands of unofficial service. Korea has a familism tradition but provides a few policies for the unofficial care-giving and trys to decrease the benefits on unofficial care. In this situation, unofficial care-givers nowadays are suffered by financial and physical matters. Therefore, support system for the unofficial care-givers is needed, such as consultation, education, and so on.
