비자살적 자해청소년의 자해중단 경험연구

본 연구의 목적은 비자살적 자해청소년들이 경험하는 자해중단과정과 중단요인을 탐색하고자 하는 것이다. 이를 위해 자해경험이 1년 이상 지속된 청소년들로서 3개월 이상 반복자해가 나타나지 않는 18명의 청소년을 선정하여 심층면접을 실시하였다. 수집된 자료는 근거이론을 적용하여 분석하였고, 분석결과 161개의 개념과 54개의 하위범주, 19개의 범주가 도출되었다. 연구결과를 요약하면, 참여자들은 고통을 견디기 위해 자해를 시도하였고, 시간이 갈수록 자해행동에 중독적으로 빠져들었다. 참여자들은 그만하고 싶어도 멈출 수 없을 정도로 중독되어 버린 자신의 상태와 죽음에 대한 공포 및 자해가 미래에 미칠 부정적 영향을 걱정하며 자신이 처한 위기를 인식하였다. 위기상태를 자각한 참여자들은 자해중단을 다짐하며 전문적인 치료를 시작하였고, 자해 외의 다른 대처방법을 찾아 시도하는 노력을 보여주었다. 참여자들의 자해중단과정은 시간의 흐름에 따라 연속적으로 진행되었는데, 반복적 자해몰입단계를 시작으로, 위기 인식단계, 치료의지 수용단계, 자해중단 실천단계, 수용적 내면 확장단계에 이르기까지 총 5단계의 과정으로 진행되었다. 이러한 연구결과를 토대로 자해청소년들의 자해중단과정에 대한 논의와 시사점을 제시하였다.

Abstract

The purpose of this study is to explore in-depth the meaning of self-injury experienced by non-suicidial self-injury youth and the process of interruption and the factors of discontinuation. To this end, this study conducted in-depth interviews with 18 youths who stopped self-injuried for more than three months among youths who had experienced non-suicidal self-injury for more than one year. The collected data were analyzed by the grounded theory. As a result of analyzing the data, 161 concepts, 54 subcategories, and 19 categories were derived. Summarizing the results, participants in this study attempted self-injury to endure pain and became addicted to self-injury behavior over time. Participants realized the addiction of self-injury and recognized the crisis they were facing. They realized the reality of the crisis by worrying about their unstoppable addiction, the fear of death, and the negative impact on the future. The participants who realized the crisis started professional treatment to stop self-injury behavior and showed efforts to find other coping methods other than self-injury. Participants' self-injury interruption consisted of a total of five stages, starting with the repeated self-injury immersion stage, the crisis awareness stage, the acceptance stage of willingness to treat, the practice stage of self-injury discontinuation, and the receptive internal expansion stage. Based on the results of this study, discussions and implications regarding self-injury interruption process of non-suicidal self-injury youth were suggested.
