본 연구의 목적은 한국의 지역 사망력과 지역 특성 간 연관성을 심층적으로 분석하고 이를 바탕으로 한 함의를 제시하는 것이다. 이를 위하여 공간적 상호의존성을 기본적으로 고려하고, 추가로 지역별 연관성이 상이함을 고려하는 계량모형을 적용하여 분석의 정교함을 보다 제고하고자 하였다. 단위 인구 당 사망률과 지역 간 연령 구조의 차이를 배제하는 연령표준화사망률을 각각 종속변인으로 고려하였을 경우 독립변인별 추정계수는 일부 변인을 제외하고 큰 차이는 나타나지 않았다. 대체로, 1인가구비율, 고령인구비율, 남녀성비와 같은 지역의 인구사회학적 특성은 사망률 및 연령표준화사망률과 정적(+) 연관성을 띠는 것으로 나타났으며, 단위 인구 당 의료기관 종사 의사 수와 1인당 보건분야 예산은 지역의 사망률을 감소시키는 것으로 도출되었다. 또한 다중범위지리가중회귀모형 적용 결과, 이러한 연관성은 지역별로 상이한 것으로 나타났다. 본 연구의 분석결과는 지역의 사망률과 지역 특성 간 연관성이 지역별로 상이하며, 그 상이함은 영향의 정도 및 방향에서도 다를 수 있음을 보여주고 있다. 이러한 분석결과는 지역 측면에서의 사망률 및 건강 불평등과 관련된 중앙정부의 정책 대응이 보다 유연하고 세부적인 방향을 견지하여야 한다는 시사점을 제시하고 있다.
Abstract
The purpose of this study is to provide the relationship between regional mortality and regional characteristics in Korea and to suggest implications. For the empirical approach, we employed an econometric model that basically considers spatial interdependence and further considers the differential relationship across regions. Considering deaths per unit population and age-standardized mortality rates, which exclude differences in age structure across regions, as dependent variables, respectively, regional socio-demographic characteristics such as single-person household ratio, elderly population ratio, and male-to-female ratio were found to be positively associated with mortality rate and age-standardized mortality rate, and the number of doctors working in medical institutions per unit of population and per capita health sector budget was derived to reduce regional mortality rates. The results show that the relationship between regional mortality and regional characteristics varies across regions, and that the magnitude and direction of the effect may also vary. Policy implications were presented based on the analysis results. They include central government policy responses to mortality and health inequalities at the local level should be more flexible and targeted.
