Abstract
The purpose of this study was to identify factors influencing healthcare utilization among persons with disabilities, focusing on healthcare accessibility at both the individual and regional levels. This study aimed to examine regional disparities and provide evidence for institutional improvements. Healthcare accessibility was categorized into four dimensions: accessibility, availability, affordability, and acceptability. Each dimension was analyzed using Andersen’s behavioral model, which incorporates both individual-level and regional-level factors. For the analysis, data from the 4th wave of the Disability and Life Dynamics Panel were combined with regional-level datasets from 2021, and multilevel modeling was employed. The results revealed that, among individual factors, distance to healthcare facilities, receipt of medical aid, and healthcare providers’ understanding of disability significantly affected outpatient service utilization. At the regional level, the number of alternative transportation options influenced outcomes at the city/county/district level, whereas the number of medical institutions had an impact at the metropolitan/provincial level, indicating variations depending on the level of analysis. Also, disability-related characteristics were found to be significant factors influencing healthcare utilization, demonstrating regional variations. These findings suggest that, to ensure the right to health for persons with disabilities through equitable healthcare utilization, it is necessary to reconstruct healthcare accessibility from the perspective of persons with disabilities and to explore support measures across its multiple dimensions.
