의료이용에 대한 저소득층의 경험과 인식 연구: Q방법론 접근

본 연구의 목적은 저소득층을 대상으로 의료이용에 대한 주관적 경험과 인식을 Q 방법론을 통해 탐색하는 것이다. 우선 Q모집단 구성을 위해 80여명의 저소득층 대상자와 인터뷰, 의료진, 전문가 자문, 관련선행연구와 문헌을 조사하여 총 65개 Q 모집단을 추출하였고, 최종적으로 33개의 진술문을 Q 표본으로 선정하였다. P 표본은 대학병원 이용 경험이 있는 저소득층 30명을 편의 표집 하였다. 수집된 자료는 PC의 QUANL 프로그램을 이용하여 분석하였다. 분석 결과 6가지 유형이 도출되었으며, 각각 1유형: 의료보다 생계우선형, 제2유형: 의료이용에 거리감형, 제3유형: 정책개선 우선 요구형, 제4유형: 소극적 질병 대처형, 제5유형: 적극적 의지와 요구 유형, 제6유형: 의료이용은 사치형 로 유형화 하였다. 결과적으로 저소득층의 의료이용에 대한 주관적 경험과 인식을 파악함으로써 향후 효율적인 보건의료체계 개선에 기초자료로 활용될 수 있으리라 기대한다.
Abstract
Introduction: Health disparities is one of the major issues for health care system in Korea. In this study, we explore the low income patient’s subjective experience and perception of medical care utilization through Q methodology. Method: Initially 65 Q samples were drawn from literature reviews and In-depth interviews for 80 low-income patient, and 33 Q statements were confirmed through staff discussion, professional consult, and pilot tests. We ask the finalized Q statements to 30 low-income patients in one university hospital in Kwang-ju who participated in the survey. Result: We found that Koreans low-income patient perceive medical utilization for six different types. First, “Living is first”, second, “Fear of complex procedures”, third, “Health care policy complaints”, fourth, “Passive response type”, fifth, “Actively service requests type”, sixth, “Medical use avoidance type”. Conclusion: We expect that this outcome will be utilized for basic data of grasp challenging aspects or difficulties when the low income bracket uses health care service and much improvement.
