We describe a man with sudden onset of one-and-a-half syndrome with bilateral lower facial palsy of motor neuron type. MRI revealed a descrete lesion in the pontine tegmentum, which provided compatible clinicoanatomical correlation of the case. We designate this neurologic finding as ‘fifteen-and-a-half syndrome’(bilateral seventh cranial nerve palsy plus one-and-a-half syndrome). As far as we know, this syndrome has never been reported.
