Background : Striatocapsular infarction(SCI) is a well-known form of stroke, which produces various neurologic symptoms including cortical dysfunction. The aim of our study was to evaluate clinical and radiological findings of SCI according to the presence of absence of cortical dysfunctions. Methods : We analyzed 44 patients with SCI and divided them into two groups according to their initial clinical manifestations. Those who showed cortical symptoms and signs were regarded as cortical type SCI and the others were classified as non-cortical type. We compared the stroke risk factors, laboratory data, clinical features and MRI/MR angiography findings between the two groups. Results : Patients with cortical type SCI and those with non-cortical type SCI had similar vascular risk factors, laboratory data and MRI findings. Cortical type SCI patients had more severe initial neurologic manifestations(Both NIHSS score and Barthel index) than non-cortical type SCI. However, there was no specific difference in their degree of clinical improvement. Regarding MR angiography findings, cortical type SCI patients had MCA stenosis more often than non-cortical type SCI did whereas non-cortical type SCI patients showed heterogeneous MR angiography findings. Conclusion: SCI with similar shape and location may have different clinical manifestations(cortical or non-cortical) according to the underlying vascular lesions.
