골내 결절종은 흔하지 않은 양성종양으로 장관골에서 주로 발견되는 것으로 육안적으로나 조직학적으로 연부조직에서 발생한 결절종과 비슷하며 병리소견으로 뮤신 물질이 섬유막으로 둘러싸인 모습으로 나타나 고 조직학적으로 섬유아세포와 점액질을 관찰할 수 있다. 저자들은 2년 전부터 외상의 과거력 없이 점진 적인 둔부 동통과 관절염 증상을 호소하는 70세 여성에서 결절종을 경험하였는데 이학적 검사에서 고관 절의 운동범위가 중등도 감소되었으며 방사선학적으로 대퇴골두의 퇴행성 변화와 함께 골두내에 경화된 경계 속에 투명한 명암을 볼 수 있었으며, 수술 시 대퇴골두 내에서 점액성 물질과 함께 조직검사에서도 결절종으로 확진되었다. 환자분이 고령이고 심한 고관절부 동통으로 인공 고관절 전치환술을 시행하였다.
Abstract
Intraosseous ganglion cysts are uncommon, benign, and usually found within long bone. The lesions appear similar to their soft tissue counterparts grossly and histologically, exhibiting a thick fibrous wall and being filled with a slimy, mucin rich, gelatinous material. A 70-year-old woman presented with a 2-year history of progressive hip joint pain with osteoarthritis. Hip joint motion was moderately limited and X-ray findings revealed a lucent area surrounded with sclerotic margin in the deformed femoral head with hip joint degenerative change. Intraoperative findings showed mucinous material in the lesion. Biopsy confirmed intraosseous ganglion. Treatment consisted of total hip replacement arthroplasty because of old age and severe hip joint pain.
