Abstract

Purpose: This study was undertaken to identify the factors that predispose to dislocation after primary total hip arthroplasty. Materials and Methods: Of 482 total hip arthroplasties, 19 patients who had at least one dislocation were compared with a control group of nineteen patients without dislocation, matched for age, weight, gender, diagnosis and design of implant. Both the clinical parameters and mechanical parameters were evaluated. Results: Dislocation after total hip arthroplasty had occurred in nineteen hips (3.9%). The mean age of patients in the dislocation group (57.1 years) was significantly higher than that in the non dislocation group (50.1 years) (p<0.05). The mean anteversion of acetabulums in the dislocation group (19.8 degrees) was lower than that in the non dislocation group (22.1 degrees) (p>0.05). There were no statistically differences among the other mechanical parameters. The dislocation risk was 8.3 times higher in patients with high ASA scores (p>0.05), 3 times higher in patients with diminished motor function (p>0.05), and 6.4 times higher in patients who were operated within the first 5 years (p>0.05). However, the total combination of clinical and mechanical risk factors significantly affected dislocation after total hip arthroplasty (p<0.05). Conclusion: A combination of multiple risk factors of clinical and mechanical parameters, rather than an isolated risk factor, significantly contributes to dislocation after primary total hip arthroplasty.
