Abstract
The authors report a case of discrepancy between results of FDP and D-dimer presented from a chronic renal failure patient. In general, a strong positive correlation is observed between FDP and D-dimer values in clinical conditions, and it has been considered that secondary fibrinolysis is the rnain event seen in the hyperplasmic state. However, in this case, there was a discrepancy in the values of the serum FDP(above which was 80μ gj mL) and the plasma D-dimer(which was 1.93 µg⁄mL). But the plasma FDP value was less than 5 µg⁄mL, which was normal. We focussed on the fibrinolytic states of this discrepancy case and tried to elucidate the mechanism of the dissociation between FDP and D-dimer values. It seems that the possible reasons for the discrepancy between FDP and D-dimer values was due to the activity difference in the fibrinogenolysis and the secondary fibrinolysis, accelerated fibrinogenolysis by decrease of α 2PI activity47%) and⁄or increase of HNE activity(243.0 µg⁄mL).
