초록

Objectives: This study reports the long-term management of two patients with essential tremor (ET) who presented with different clinical phenotypes using a complex Korean Medicine protocol. Methods: Two patients diagnosed with ET underwent treatment with herbal medicine (Eokgansan-ga-jinpi-banha) and acupuncture, with pharmacopuncture in one case. Primary outcome was Patient Global Impression of Change (PGI-C). Secondary outcomes were assessed using a Visual Analog Scale (VAS) for tremor intensity, ET Rating Assessment Scale for Activities of Daily Living, and the Quality of Life in ET questionnaire. Results: Both patients showed significant improvement. Case 1 (stress-sensitive type) showed VAS reduction from 5 to 0 with PGI-C rated "Much improved." Case 2 (late-onset, action-induced type) demonstrated VAS reduction from 10 to 0-1, maintained with PGI-C "Very much improved." ADL and quality of life score improved in both patients. Conclusions: These cases suggest a phenotype-guided complex Korean Medicine protocol may be an effective strategy for the long-term management of ET. This personalized approach appears to control motor symptoms and their underlying variability, improving quality of life.
