Abstract

Background_Diplopia is a common manifestation of myasthenia gravis (MG), with over half of patients presenting with ocular symptoms such as diplopia and ptosis. Ocular myasthenia gravis (OMG) can mimic isolated extraocular muscle palsy, potentially leading to delays in clinical diagnosis. As such atypical manifestations complicate the diagnostic process for neurologists, a careful differential diagnosis is necessary. Case report_A 63-year-old man presented with horizontal diplopia that had developed 4 months earlier. Neurological examination revealed a limitation of right eye abduction without pain or diurnal fluctuation. Initial blood tests, thyroid function tests, ganglioside antibody tests, and brain MRI were unremarkable, leading to a diagnosis of microvascular abducens nerve palsy. However, as symptoms remained unchanged after 4 months, reevaluation was performed. Testing revealed positive acetylcholine receptor antibodies and an abnormal decremental response on repetitive nerve stimulation, confirming the diagnosis of OMG. Treatment with prednisolone led to complete resolution of diplopia within 8 weeks. Conclusion_Although isolated abducens nerve palsy is most often caused by microvascular ischemia, OMG can rarely present in a similar manner. This case highlights an atypical form of OMG without diurnal fluctuation or fatigability, initially misdiagnosed as abducens nerve palsy. Clinicians should consider OMG in the differential diagnosis of painless, persistent extraocular muscle palsy.
