자궁경수술로 성공적으로 치료된 점막하 낭성자궁선근종 2예

자궁선근종은 조직학적으로 자궁내막조직이 자궁의 근층을 침범한 것을 특징으로 하는 질환이다. 자궁선근종은 혈액이 차 있는 작은 낭성구조로 보이기도 하나, 점막하에 위치하는 큰 낭성종괴로 보이는 경우는 매우 드물다. 저자들은 이상 질출혈을 보인 환자에서 시행한 질식초음파상 자궁의 내막부위에 종괴가 보여 식염수 주입 초음파자궁조영술을 시행, 점막하자궁근종의 낭성변성으로 생각되어 자궁경하 종괴제거술을 시행한 환자에서 조직학적으로 점막하낭성자궁선근종으로 진단된 2예를 경험하였기에 간단한 문헌고찰과 함께 보고하는 바이다.
Abstract
Two women presented with history of vaginal bleeding and abnormal transvaginal ultrasound findings. Saline infusion sonohysterography was done for preoperative evaluation and this imaging study revealed centrally located uterine mass with internal cystic portion. Our initial impression was submucosal myoma with cystic degeneration and hysteroscopic removal of the mass was performed. During the procedure, dark brownish cystic fluid was drained. The excised submucosal mass was pathologically diagnosed as adenomyosis and clinically as submucosal adenomyotic cyst.
