만성질환 노인을 돌보는 가족부양자의 건강관련행동변화에 관한 연구

초록

본 연구는 스트레스와 대응모델을 근거로(Lazarus and Folkman, 1984) 만성질환노인을 모시는 가족부양자가 부양으로 인해 경험하는 부양부담과 부양만족감을 파악하고, 부양자의 건강관련행동변화에 미치는 직․간접적 영향을 규명하기위해 실시되었다. 이를 위해 대구․경북지역에 거주하는 65세 이상의 만성질환으로 인해 일상생활이 어려운 노인을 모시고 사는 가구의 여성 233명을 대상으로 설문조사를 실시하였으며, 그 결과를 SPSS 프로그램을 사용하여 분석하였다. 분석결과, 노인의 부양기간과 부양과업, 부양자의 경제수준이 가족부양자의 부양부담에 영향을 미치는 유의한 변인으로 나타난 반면, 부양자의 부양만족감에 유의한 변인으로는 부양과업과 경제수준으로 확인되었다. 이러한 연구결과는 노인을 돌보는 가족부양자의 부양부담과 부양만족감은 단순히 강한 부적관계를 가진 동일한 속성을 지닌 변인이 아니라 관련성은 지니나 별개의 변인이라는 결과를 제시하고 있다. 또한 부양자가 경험하는 부양과업과 부양부담은 부양자의 건강관련행동변화에 유의한 부적 영향을 미치는 것으로, 부양자의 경제수준과 부양만족감은 부양자의 건강관련행동변화에 유의한 정적영향을 미치는 것으로 제시되었다. 특히 부양부담과 경제수준은 부양자의 건강관련행동변화에 직접적 효과 뿐 만아니라 부양부담과 부양만족감을 통한 간접적 효과를 가지는 것으로 확인되었다. 마지막으로 본 연구결과에 따른 실천적 함의와 제언이 논의되었으며, 만성질환노인과 가족부양자를 대상으로 하는 후속연구에 대한 방안을 제시하였다.

Abstract

This study investigates how providing cares to the elderly with chronic disease effects family caregivers' health related behavior change based on stress and coping model. For the purpose of the study, the survey was conducted and the respondents were 233 family caregivers who reside with the frail elderly over 65 years old living in Daegu and Kyongpook province. The result of the study indicated that caregiving tasks, length of caregiving, and financial status were significant variables in explaining caregiving burden, while caregiving tasks and financial status were significant in explaining caregiving satisfaction. Thus length of caregiving, caregiving tasks and financial status were analyzed to find out how providing cares impact on caregivers' health related behaviors. The results showed that caregiving tasks and financial status were significant factors in explaining caregivers' health related behavior change directly and indirectly. Both factors were more influenced directly health related behavior change directly than indirectly through caregiving stress and satisfaction. Finally, based on the findings of the study implications for the elderly cares and recommendations of further study were provided.
