국내 장애정도 판정기준 개선을 위한 고찰: 발달장애(지적장애 및 자폐성장애) 중심으로

초록

국내에 등록한 발달장애인의 수가 늘면서 복지서비스 또한 확장되고 있으며, 서비스의 첫 단계인 발달장애 진단 및 판정 절차가 전문적이고 엄정하게 이루어져야 할 중요성이 제고된다. 이에 국내 장애정도 판정에서 발달장애 관련 정책과 실제를 고찰하고 보완이 필요한 부분을 논의하였다. 먼저 정책적 패러다임부터 현황과 요구, 현행 판정기준과 이에 포함된 심리검사, 판정의 행정적 절차와 지원 내용을 살펴보았다. 이후 타당하고 신뢰로운 판정을 위해 현행 기준에서 보완이 필요한 부분을 세 가지로 논의하였다. 첫째, 판정기준에 발달장애의 최신 개념과 더불어 핵심 요소를 균형적으로 반영해야 하며, 지능지수 단독이 아닌 적응행동 기능과 핵심증상이 더 종합적으로 고려될 필요가 있다. 둘째, 판정 시 특정 검사의 단일 수치에 따라 분류하기보다는, 다양한 정보원과 출처에 전문가의 임상적 판단이 통합된 종합적 해석과 감별진단이 더 고려될 필요가 있다. 셋째, 근거기반 심리평가를 위해 실시자 자격 요건이 규정될 필요가 있다. 향후 발달장애 판정에서 정확한 평가와 감별진단, 나아가 적극적인 관련 실무 및 연구에 대해 임상심리학자의 활발한 논의가 있기를 기대한다.

Abstract

We reviewed the current paradigm and issues around the Korean disability registration policy, service contents, as well as the determination guidelines, including the appropriate use of the psychological measures and comparison of diagnostic systems (i.e., DSM and ICD). Subsequently, we discussed three tenets to ensure the validity and reliability of the disability determination procedures: First, it is critical to follow diagnostic guidelines given by the latest editions of the diagnostic systems. Second, evidence-based psychological and comprehensive assessments should integrate multiple sources of information, which can maximize clinical judgment for differential diagnoses. Third, the assessment of the disability service provision, which includes administration and interpretation of psychological tests, should be carried out by qualified professionals, and the qualification requirements need to be specified more clearly. We expect this article could encourage clinical psychologists’ active involvement in the practice and research for the Korean developmental disability determination process.
