배경 : 자가면역성 혈소판 감소증(AITP)에서 혈소판의 당단백에 대한 IgG 항체가 병인에 중요하다. 그러나 이에 대한 측정은검출 방법이 복잡해 널리 이용되지 못하고 있다. 저자들은 AITP진단을 위한 혈소판 당단백 특이항체(PLT GPsAb)의 임상적 유용성을 살펴보았다.방법 : 혈소판 수가 10 × 109/L 이하이며 골수검사를 시행한환자 57명을 대상으로 하였다. 골수검사 결과와 임상기록 분석을통해 구분된, 일차성 혈소판 감소증(primary AITP, P-AITP) 환자 21명, 이차성 혈소판 감소증(secondary AITP, S-AITP) 환자 15명, 골수에서 생산이 감소된 비면역성 혈소판 감소증(non-immune, NITP) 환자 21명을 포함하였다. Modified AntigenCapture ELISA test (MACE, GTI, USA)를 이용하여, 냉동보관된 혈장검체 내에서 항-GP IIb/IIIa, Ia/IIa, Ib/IX, IV 항체4가지와 항-HLA class I 항체를 검출하였다. 결과 :총 57예 중 PLT GPsAb는 AITP 환자의 22.2% (8/36),NITP 환자의 4.8% (1/21)에서 양성을 보였다. PLT GPsAb 양상은, GP IIb/IIIa 4예, GP Ia/IIa 5예, GP Ib/IX 3예, GP IV2예였다. PLT GPsAb 양성 9예 중 4예에서 2가지 이상의 PLTGP에 대한 항체 양성이었다. P-AITP와 S-AITP 사이에 PLT-196 김현정오은지김자영 외 2인sAg 양성률의 유의한 차이는 없었다(23.8% vs. 20.0%, P>0.05).AITP 진단을 위한 MACE 검사의 민감도, 특이도, 양성예측률,음성예측률은 각각 22.2%, 95.2%, 88.9%, 41.7%이었다. 수혈 기왕력이 있는 환자에서 항-HLA class I 항체의 양성률이 높았다(P<0.05).
