초록

Backgroud : Monoparesis is characterized by prominent motor weakness limited to one limb. Although rare, stroke or other cerebral lesions may produce monoparesis. Methods:Eight stroke patients with suddenly developed monoparesis underwent CT and MRI. We analyzed clinical and radiological features, and the results were correlated. Results : Of eight patients, seven had upper extremity weakness and one had lower extremity weakness. Among patients with upper extremity weakness, primary motor cortex infarction was shown in five, postcentral gyrus in one and centrum semiovale infarction in one. Lesions producing lower extremity weaknees were present in paraventricular white matter posterior portion. Five of eight patients showed increased DTR in the distribution of weakness(biceps jerk: 4, triceps jerk: 2, knee jerk: 1).Two patients had sensory abnormalities (pain and vibration), below the wrist. Conclusion : Lesions producing monoparesis are usually localized in the cortex. For patients with sudden onset of monoparesis, careful neurologic examination, including DTR and an imaging study such as an MRI are needed. Korean Journal of Stroke 2002;4(1): 25~29
