안허혈 증후군을 동반한 급성 망막중심동맥폐쇄의 치료 1예

초록

목적: 단안에 발생한 안허혈 증후군을 동반한 급성 망막중심동맥폐쇄의 치료 1예를 경험하였기에 이를 보고하고자 한다. 증례요약: 72세 남자 환자가 내원 당일 갑자기 발생한 우안 시력 저하를 주소로 본원 내원하였다. 내원 당시 우안 시력은 광각유, 좌안은 1.0이었다. 우안 안저검사상 시신경 유두부위 망막혈관에 큰 색전 및 중심와 부위 앵두반점이 관찰되었고, 형광안저혈관조영술상맥락막 충만이 전반적으로 지연되어 있었으며 심한 망막동정맥통과시간의 지연이 관찰되었다. 경동맥 혈관조영술상 우측 경동맥에심한 협착소견을 보였다. 혈액응고이상을 포함한 전반적인 혈액학적 검사를 시행하였으며, 특이소견은 보이지 않았다. 우안 안허혈증후군을 동반한 급성 망막중심동맥폐쇄 진단하에 경혈관 Nd:YAG 레이저 색전제거술 2차례 시행하였으며, 우측 경동맥 협착부위에스탠드 삽입술을 시행하였다. 치료 후 한 달째 우안 시력 0.06으로 시력 회복을 보였으며, 형광안저혈관조영술상 망막동정맥통과시간의 지연소견은 보이지 않았다. 결론: 망막중심동맥폐쇄와 함께 맥락막 혈류장애가 동반되는 경우 시력 예후가 매우 불량한 것으로 알려졌으나, 본 저자들은 안허혈증후군을 동반한 망막중심동맥폐쇄 환자에서 신속한 Nd:YAG 레이저 색전제거술 및 경동맥 혈관성형술 및 스텐트 삽입술을 통해 성공적인 치료를 경험하였기에 이를 보고하는 바이다.

Abstract

Purpose: We report a case of treatment of acute central retinal artery occlusion (CRAO) with ocular ischemic syndrome (OIS). Case summary: A 72-year-old man presented with acute loss of vision in the right eye on that day. At initial examination, visualacuity tested positive for light sensitivity in the right eye. Fundus examination demonstrated a visible embolus at the central retinalartery overlying the optic disc head and a cherry-red spot in the fovea. Fluorescein angiography revealed that filling of thechoroidal circulation was delayed, and the arteriovenous transit time was even further delayed. Carotid angiography showedmarked stenosis within the right internal carotid artery. Laboratory tests included blood tests for hypercoagulability evaluation, forwhich the results were non-specific. To treat acute CRAO with OIS in the right eye, transluminal Nd:YAG laser embolectomy(TYE) was performed twice, and carotid angioplasty with stenting was conducted within the stenosed internal carotid artery. Onemonth after the TYE procedure and carotid stenting, the patient’s visual acuity improved to 0.06 and the arteriovenous transittime was within normal limits on fluorescein angiography. Conclusions: The visual prognosis in eyes with CRAO plus an associated choroidal circulatory disturbance is extremely poor. However, we experienced and reported a case of CRAO with OIS treated successfully through a prompt TYE procedure and carotidangioplasty with stenting.
