Abstract
The accelerated global aging trend has underscored the need for early detection and prevention of frailty. The oral cavity serves as a highly sensitive biological sensor of systemic health, capable of detecting early signals of systemic inflammation, metabolic dysregulation, immunosenescence, and neurodegenerative processes through non-invasive samples such as saliva, gingival crevicular fluid (GCF), and the oral microbiome. Oral frailty has gained attention as a strong predictor not only of impaired mastication and swallowing, but also of malnutrition, sarcopenia, cognitive decline, cardiovascular disease, and Alzheimer’s disease. Oral aging is marked by epithelial thinning, connective tissue remodeling, and reduced salivary gland function, while the transition to oral frailty involves mitochondrial dysfunction, chronic inflammation, oxidative stress, and diminished regenerative capacity. These changes reflect broader systemic alterations and are closely interconnected with the development of whole-body frailty. This review summarizes current frailty diagnostic frameworks and emerging oral biomarkers, and highlights the potential of the oral cavity as a platform for early systemic frailty detection. Oral-based early intervention strategies may ultimately support frailty prevention, personalized healthcare, and the extension of healthy lifespan.
