미충족 의료 경험과 보건의료체계에 대한 인식

본 연구는 일반 국민이 주관적으로 인식하는 미충족 의료 현황의 추이를 분석하고 이와 관련된 요인을 파악하며, 미충족 의료 경험이 보건의료체계에 대한 인식과 어떤 연관성을 가지는지를 살펴보는 것을 목적으로 한다. 2011년∼2013년 3년 연속 한국의료패널조사를 완료한 패널을 대상으로 확률효과 패널 로짓 모형을 사용하여 분석하였다. 미충족 의료를 경험한 사람의 비율은 2011년 14.89%에서 2013년 17.64%로 증가하였고, 경제적 부담으로 인한 미충족 의료 경험과 시간적 부담으로 인한 미충족 의료 경험은 대체관계에 있음을 확인하였다. 미충족 의료 경험에 영향을 미치는 요인을 분석한 결과, 여성이거나, 45~54세의 중장년층, 초등학교 이하의 학력을 가진 경우, 소득이 낮은 경우, 지역건강보험이거나 의료급여인 경우, 주관적 건강이 나쁜 경우에 미충족 의료 경험 가능성이 유의하게 높았다. 미충족 의료 경험은 높은 의료비 본인부담을 보건의료체계의 문제로 인식할 가능성과 연관이 있고, 의료서비스의 질적 수준이 우수하다고 답할 가능성을 낮추었으며, 의료체계가 잘 작동한다고 평가할 가능성을 낮추어, 전반적으로 보건의료체계에 대한 부정적인 인식과 관련이 높음을 알 수 있다. 본 연구에서는 미충족 의료 경험과 보건의료체계에 대한 인식의 연관성을 확인하였다. 앞으로 미충족 의료의 다차원적 성격을 탐색하고 이를 보건의료체계의 반응성 개념과 연계하여 반응성에 대한 종합적인 평가가 이루어져야 할 것이다.

Abstract

The objectives of this study was to investigate unmet needs for health care and to explore attitudes towards health care system and association with unmet needs. Data from the Korea Health Panel Survey using nationally representative sample was analyzed.. To identify factors associated with unmet needs and attitudes towards health care system multivariate logistic regression analyses using balanced panel data from 2011 through 2013 were employed. Respondents who experienced unmet needs for health care increased from 14.89% in 2011 to 17.64% in 2013. Those with disadvantaged socioeconomic status were more likely to experience unmet needs were and there was significant association between experiences of unmet needs and less supportive attitudes towards health care system. Findings suggest unmet health needs may be useful to investigate responsiveness of health care system.
