EQ-5D를 이용한 강박장애 환자의 삶의 질과 증상 중증도의 연관성 연구

강박장애는 평생 유병률이 2~3%1) 정도로 흔한 정신질환으로, 환자들은 자신의 의지와는 무관하게 특정한 생각이나이미지, 충동을 반복 경험하고 이를 감소시키기 위한 강박행동을 반복한다. 대부분의 경우 10~20대에서 발병하지만30~40대에서도 발병할 수 있고, 발병 이후 상당수의 환자에서만성적인 경과를 보이기 때문에,2) 적극적인 사회생활이 필요한시기에 많은 영향을 받게 된다. 여러 편의 이전 연구를 통해강박장애에서의 삶의 질 저하가 잘 알려져 있다.3) Moritz 등4) 은 강박장애에서의 삶의 질 연구들을 리뷰한 연구에서 강박장애가 환자들에서 유의한 삶의 질 저하를 일으키고, 증상의중증도가 미치는 영향을 분석하였다. 또한 Angst 등5)은 591 명의 대상자들을 15년 이상 추적 관찰한 연구에서 강박증상의 정도에 비례하여 삶의 질이 감소함을 보고하였다. 그러나국내에서는 강박장애의 삶의 질과 관련하여 소수의 연구만존재하고 있다. 한편, 의료분야에서 중요한 건강관련 삶의 질(health-re-lated quality of life, HRQoL) 평가 방법에는 ‘일반적(generic)’ 삶의 질 평가 도구와 특정 질환의 삶의 질을 평가하기위한 ‘질병특이(disease-specific)’ 삶의 질 평가도구로 나눌수 있다.6) EuroQoL 그룹의 전문가들은 특정 질병에 국한되지 않은 일반적 삶의 질 평가도구인 Euro-QoL-5D(이하EQ-5D)를 개발하였다.7) EuroQoL에서 개발된 EQ-5D에는 3 단계 응답 버전(EQ-5D-3L)과 5단계 응답 버전(EQ-5D-5L) 이 있다. EQ-5D-5L는 운동능력(mobility), 자기관리(selfcare), 일상활동(usual activities), 통증/불편감(pain/discomfort) 과 불안/우울(anxiety/depression)의 5개 항목에 대하여5단계로 자가 보고하는 간단한 삶의 질 측정 도구이다.7)8) EQ-5D의 특징은, 각 나라별 가치평가 세트(valuation set)에의하여 단 하나의 숫자로 표현되는 평가점수로 환산할 수 있다는 점이다. 이 평가점수를 이용하면 질 보정수명(qualityadjusted life years, QALYs)을 구할 수 있기 때문에 많은 국가에서 널리 쓰이고 있다. 한국에서도 2016년 EuroQoL 그룹의 표준화된 프로그램에 의한 가치평가 연구가 이루어졌다.8) 즉 EQ-5D를 사용한 삶의 질 연구는 일반인구집단의 측정치나 다른 신체질환 및 정신장애에서의 삶의 질을 비교할 수 있다는 장점을 가진다.9)10) 그러나 국내의 경우 강박장애 환자에서의 삶의 질이 The World Health Organization Quality of Life-BREF(이하 WHOQOL-BREF) 등을 이용하여 연구된 적은 있으나 EQ-5D를 이용하여 연구된 바는 없다.11) 이전 연구에서, 강박장애 환자의 삶의 질을 다른 건강문제와 비교할경우, 환자의 모집방법과 증상의 중증도가 영향을 미치게 된다는 점이 알려져 있다.4)12) 본 연구에서는 강박장애 환자들을 대상으로 EQ-5D 평가를 통하여 환자의 강박장애가 삶의 질에 미치는 영향과, 증상중증도가 미치는 영향에 대하여 측정하고자 한다. 이를 통하여 향후 의료 자원 혹은 사회적 자원의 배분에서의 근거가 될수 있는 기본 자료가 구축되도록 하고자 한다.
Abstract
Objectives : Obsessive-compulsive disorder (OCD) is a disabling psychiatric disorder, and more attention is recently paid on the quality of life (QoL) in OCD patients. The Euro-QoL-5D (EQ-5D) is a widely used self-report to calculate a single score which represents ‘health utilities’. The aim of this study was to assess the health-related QoL for patients with OCD using the EQ-5D and to examine the relationship between health-related QoL and symptom severity. Methods : Seventy-three patients with a Diagnostic and Statistical Manual of Mental Disorders (DSM-5) diagnosis of OCD were recruited from the outpatient clinic in Seoul National University Hospital. Symptom severity was assessed using the Yale-Brown Obsessive- Compulsive Scale (YBOCS), and QoL was assessed with the EQ-5D-5L questionnaire. Using Korean valuation study, a single score of QoL was calculated. Results : Most of the OCD patients were relatively young (＜ 45 years) with the mean YBOCS total score of 19.33. The mean EQ-5D score was 0.71 and significantly correlated with symptom severity (r = -0.483, p ＜ 0.001). 25% of the EQ-5D score was predicted by the YBOCS total score (b = -0.011, p ＜ 0.001) by regression analysis. Conclusions : OCD patients suffer from lower health-related QoL and QoL significantly decreased as symptom severity increased. The results of the EQ-5D would enable further studies on QoL comparison across medical disease and mental disorders.
