초록

Payment systems are important in supporting the core values of healthcare systems, driving changes in delivery structures and processes. Healthcare values are defined as health improvement, responsiveness, and efficiency, with an emphasis on the important role of stakeholders in promoting these values. This study aims to analyze trends in value-based payment systems in Organisation for Economic Co-operation and Development (OECD) countries and examine their implications for the Korean healthcare system. Value-based payment models in OECD countries can be categorized into episode-based bundled payments, bundled payments for chronic disease management, and comprehensive population-based payment systems. Although evaluations of health care expenditure have varied across programs, the overall impact of these models on quality of care and patient experience has been positive. Transitioning to bundled payment systems is essential to improve the value of healthcare spending. However, a significant barrier to adopting value-based payment systems is the lack of acceptance by healthcare providers. To address this challenge, it is important to establish a consensus among stakeholders on acceptable frameworks and principles. Furthermore, a gradual implementation approach, supported by pilot programs and ongoing assessments, is necessary to ensure the successful transition from traditional payment systems to value-based models. This study emphasizes the importance of aligning stakeholder interests and adopting phased strategies to support the successful adoption of value-based payment systems.
