흉강천자 직후 제왕절개술 및 고빈도양압환기를 이용한 저항성 선천성 유미흉 태아 치료 1예

저자들은 갑상선기능저하증에 동반된 중증 선천성 유미흉 태아 1예를 경험하였다. 이 태아의 흉수는 초음파 유도에 의한 자궁 내 흉강천자술과 OK-432를 사용한 흉막유착술을 시행하였으나 반응하지 않고 다시 차올랐으므로, 저자들은 흉강천자 직후 흉수가 다시 차올라폐를 압박하기 전에 제왕절개술로 분만한 뒤 고빈도양압환기를 적용하였다. 본 증례의 경험으로부터 저자들은 선천성 유미흉의 원인적검사에 태아 갑상선기능 상태가 고려되어야 하고, 일반적인 처치에 반응을 안 할 경우 흉강천자 후 즉각적인 제왕절개술로 출생하여 신생아 처치를 하는 것이 하나의 대안이 될 수 있다고 제시하는 바이다.
Abstract
We experienced a case of fetus with congenital chylothorax who treated with thoracocentesis immediately followed by cesarean delivery with high frequency ventilation therapy after treatment failure by serial thoracocentesis and pleurodesis with OK-432. This approach achieved an effective control. So we suggest that cesarean delivery along with postnatal high-frequency ventilation immediately after thoracocentesis can be considered in a fetus with intractable congenital chylothorax.
