초록

Klebsiella pneumoniae is a significant opportunistic pathogen responsible for both hospital- and communityacquired infections. Due to the lack of available treatments, the emergence of bacteria that produce extended-spectrum beta-lactamases (ESBLs) and are multidrug-resistant (MDR) presents a serious therapeutic problem. The purpose of this cross-sectional study was to ascertain the frequency and pattern of antibiotic susceptibility of K. pneumoniae that was isolated from different clinical samples in a tertiary care facility. Conducted at Sree Balaji Medical College and Hospital over eight months (November 2023 to June 2024), the study analysed 250 clinical samples. Identification of K. pneumoniae was done using standard microbiological techniques, and antibiotic susceptibility testing was performed using the KirbyBauer disc diffusion method according to the 2024 CLSI guidelines. ESBL production was detected phenotypically using the Double Disc Synergy Test (DDST) with ceftazidime, cefotaxime, and amoxicillinclavulanic acid discs. Among the 250 samples, 84 (33.6%) yielded K. pneumoniae, with sputum being the most common specimen (41.6%), followed by urine (23.8%). Of the 84 isolates, 51 (60.7%) were ESBL producers, with a higher prevalence among inpatients (70.5%). High resistance was observed to ampicillin (70.2%) and cefazolin (46.4%), whereas significant sensitivity was noted to chloramphenicol (91.7%), amikacin (86.9%), gentamicin (86.9%), and tobramycin (84.5%). The results highlight the significant prevalence of MDR and ESBL-producing K. pneumoniae and stress the significance of consistent monitoring of antimicrobial resistance, judicious antibiotic use and stringent infection control protocols in preventing resistance transmission and guaranteeing successful clinical treatment.
