목적: 어렸을 때 외상으로 인해 맥락막 파열이 생긴 환자에서 40년 이상 경과 후 발생한 전층 황반원공을 유리체절제술로 치료한 증례를 보고하는 바이다. 증례요약: 6살 경 야구공에 좌안을 수상 당한 이후 별다른 증상이 없이 지내온 50세 남자 환자가 2년 전부터 시작된 좌안 시력 저하를주소로 내원하였다. 안저검사 상 전층 황반원공이 관찰되었고, 황반 이측으로 과거 외상으로 인해 발생한 것으로 추정되는 맥락막파열로 인한 흉터 조직이 관찰되었다. 이에 유리체절제술을 시행하였고 전층 황반원공은 폐쇄되었으며 시력은 호전되었다.
Abstract
Purpose: We report a case of a full-thickness macular hole which occurred many years after a blunt eye trauma leading tochoroidal rupture. Case summary: A 50-year-old male visited our clinic with a complaint of decreased vision in his left eye 2 years in duration. He experienced a blunt trauma to his left eye with a baseball when he was 6 years old, although he did not complain of anyvisual disturbance in the left eye at that time. Fundus examination revealed a full-thickness macular hole with vertical fibroticscar at the temporal side of the macula, which was thought to be a choroidal rupture induced by the previous blunteye trauma. We performed vitrectomy and intravitreal tamponade injection. Two months later, the full-thickness macularhole completely closed and visual acuity of the left eye improved.
