비강상태에 따른 비내시경적 누낭비강문합술의 결과 분석

목적: 비내시경적 누낭비강문합술을 시행한 환자에서 비강상태와 수술 성공과의 관계에 대해 분석하고자 한다. 대상과 방법: 2007년 1월부터 2011년 12월까지 204명 306안을 대상으로 술 전 비강내 이상 유무와 수술적 교정 유무, 술 후 비강내충전물에 따른 술 후 비강내 합병증 발생률과 각 인자들의 상관관계 및 성공률에 대해 분석하였다. 결과: 일차 수술 성공률은 87.3% (267/306)이며, 재수술, 육아종제거 등 시행 후 눈물 흘림이 소실된 이차 성공률은 90.5% (277/306)였다. 술 후 비강내 합병증은 40안에서 발생하였고, 비강내 충전물로 Nasopore를 사용한 47.8% (32/67)에서 합병증이 발생해Merocel을 사용한 3.3% (8/239)에 비해 통계적으로 유의한 증가를 보였다. 비강내 합병증 발생률은 정상비강 8.7%, 술 전 수술적교정을 한 경우 16.7%, 교정을 하지 않은 경우 20%로 비정상 비강일 경우 합병증 발생이 통계적으로 유의하게 증가하였고(p=0.019),이러한 합병증 발생은 일차 수술 성공유무에 유의한 영향을 주었다(p=0.008). 그러나 술 전 비강내 이상소견과 일차 수술 성공유무는통계적으로 유의하지 않았다(p=0.479). 결론: 비내시경적 누낭비강문합술에서 술 전 비강내 이상 유무 및 술 후 비강내 충전물의 종류는 비강내 합병증 발생에 유의한 영향을미쳤으나 수술 성공률은 영향을 미치지 않았다. 그러나 술 전 비강내 이상 교정 및 Nasopore 대신 Merocel 삽입은 상대적으로 합병증 발생을 감소시켜 환자의 만족도를 높일 것으로 생각한다.

Abstract

Purpose: This study evaluated the relationship of nasal cavity state and surgical results after endonasal dacryocystorhinostomy(DCR). Methods: We retrospectively analyzed 306 eyes of 204 patients between January 2007 and December 2011. The correlation betweenthe existence of preoperative nasal cavity abnormality, postoperative nasal cavity complications after proper managementof nasal cavity abnormality, and postoperative nasal cavity complications according to nasal packing material wereinvestigated. We analyzed the success rates and the correlation between each set of factors. Results: The success rate of primary operations performed six months after tube removal was 87.3% (267/306). The successrate after secondary revision, granuloma removal and punctoplasty was 90.5% (277/306). Postoperative nasal cavity complicationswere inspected in 40 eyes. Of the 306 eyes, patients treated with Nasopore showed significantly more postoperative nasalcavity complications (47.8%, 32/67) than in patients treated with Merocel (3.3%, 8/239). The rate of postoperative nasal cavitycomplications was 8.7% in patients with normal nasal cavity, 16.7% in patients who received treatment, and 20% in patients withouttreatment, with significant statistical increase if the nasal cavity was abnormal (p = 0.019). The incidence of complications influencedthe primary success rate (p = 0.008); however, preoperative nasal cavity abnormalities were not correlated with primarysuccess (p = 0.479). Conclusions: In the case of endonasal DCR, preoperative nasal cavity abnormality and type of nasal packing material used didnot affect the success rate but significantly affected postoperative nasal cavity complications. In conclusion, endonasal DCR withpreoperative treatment of nasal cavity abnormality and Merocel packing is expected to reduce postoperative nasal cavity complicationsand increase patient satisfaction.
