아래눈꺼풀속말림 환자에서 비흡수성 봉합사를 이용한 개선된 퀴커트 봉합술

목적: 아래눈꺼풀속말림 환자를 대상으로 비흡수성 봉합사를 이용한 Quickert 봉합술(퀴커트 봉합술)을 시도하고 이 방법의 유용성을평가하고자 하였다. 대상과 방법: 2011년 10월부터 2012년 6월까지 여러 원인의 아래눈꺼풀속말림으로 진단받은 환자 중 복잡한 수술적 치료를 원하지않거나 전신상태 저하로 수술적 치료를 시행 받기 어려운 환자 11명, 12안을 대상으로 하였다. 아래눈꺼풀 속눈썹 바로 밑 안쪽, 가운데, 가쪽 3곳에 수평 방향의 소절개를 시행하고, 비흡수성 봉합사(double armed 6-0 nylon)를 이용하여 두 개의 바늘을 아래결막구석에서 피부 절개를 실시한 부위로 나오게 통과시켜 묶은 후 그 매듭을 피부 절개부위 아래로 매몰시켰다. 이러한 봉합을 안쪽, 가운데, 가쪽 3군데 시행하였다. 결과: 평균 연령은 71.3 ± 8.4세(54-82세)였고 남자 4명, 여자 7명이었다. 수술 후 평균 13.9 ± 2.4개월간 경과관찰하였다. 모든환자에서 아래눈꺼풀속말림이 해결되었고, 추적관찰기간 동안 재발은 없었다. 결론: 퀴커트 봉합술은 그 술기로 간단하나 효과가 일시적이라는 한계가 있다. 저자들이 시도한 비흡수성 봉합사를 이용한 방법은봉합사 자체의 장력을 이용하기 때문에 장력이 유지되는 동안 그 효과가 지속되어 장기간의 교정효과를 얻을 수 있는 장점이 있어,수술적 치료를 원하지 않거나 전신상태가 나빠 수술적 치료를 시행 받기 어려운 환자들을 대상으로 입원실이나 외래에서 일차적으로간단하게 시도해볼 수 있을 것이다.
Abstract
Purpose: To introduce the modified Quickert suture for lower lid entropion, using nonabsorbable suture material. Methods: From October, 2011 to June, 2012, a total of 11 patients (12 eyes) with lower lid entropion, in poor general conditionor who did not want extensive surgery, were recruited for the present surgery. Three small skin incisions were made at the medial,central, and lateral areas, just below the lower cilia. With double armed 6-0 nylon, each needle was inserted in the inferiorconjuctival fornix and the 2 ends of the suture were tied and buried at the point of the skin incision site. Sutures were made at themedial, central and lateral areas. Results: The patients consisted of 4 males and 7 females with an average age of 71.3 ± 8.4 years (54-82 years). The patientswere followed up the patients for an average of 13.9 ± 2.4 months postoperatively. All patients were satisfied with the outcome,and there were no recurrences. Conclusions: Quickert suture is a simple and effective method, for correcting lower lid entropion by inducing scar formation withabsorbable suture materials, but the effect duration is limited. Because the modified Quickert suture utilizes its own tension witha nonabsorbable suture material, the effect lasts as long as the suture material remains. It is a useful and practicable method forpatients, in poor general condition or not wanting extensive surgical procedures.
