전신 림프종과 동반된 이차성 맥락막 림프종 1예

목적: 이차성 맥락막 림프종 1예를 경험하였기에 이를 보고하고자 한다. 증례요약: 57세 여자 환자가 1개월 전부터 발생한 우안 시력 저하를 주소로 내원하였다. 세극등 현미경 검사상 우안의 유리체내에염증세포들이 보였고, 안저 검사상 이측 부위에 노란색의 융기된 병변이 관찰되었으며, 전 사분면에 걸쳐서 황색의 반점으로 나타나는 다수의 맥락막 침윤이 보였다. 초기 진단으로서 우안의 안구내 림프종의 가능성을 고려하여 진단적 유리체 절제술을 통하여 유리체 조직 검사를 하였으나 특이 소견을 보이지 않았다. 전신 검사 결과 경부 림프절 비대가 관찰되었고, 시행한 조직 검사상 미만성거대 B세포 림프종(diffuse large B-cell lymphoma)으로 나타났다. 이에 전신 림프종에 동반된 이차성 맥락막 림프종으로 진단하고전신적인 항암치료와 함께 유리체내 methotrexate 주입술을 4회 시행 후 경과 관찰 중이다. 결론: 맥락막 종괴가 전신 림프종의 첫 증상으로 나타날 수 있으므로 전신적인 검사를 통한 림프종에 대한 평가가 필요하다.
Abstract
Purpose: To report a case of secondary choroidal lymphoma. Case summary: A 57-year-old female presented at our clinic with decreased vision in her right eye 1 month in duration. Slit-lampexamination showed vitreous cells in the right eye. Fundus examination of the right eye revealed an elevated yellowish mass-likelesion at the temporal area and multifocal yellowish patches at the choroidal level. The clinical impression was intraocularlymphoma. We performed diagnostic vitrectomy, but the result was negative. A systemic evaluation revealed enlarged cervicallymph nodes and lymph node biopsy showed diffuse large B-cell lymphoma. The patient was diagnosed as secondary choroidallymphoma associated with systemic lymphoma and was treated with systemic chemotherapy and 4 injections of intravitrealmethotrexate. The patient is scheduled for regular follow-ups. Conclusions: Choroidal mass may be the initial presenting sign of systemic lymphoma, and an extensive systemic evaluationshould be performed.
