만성 중심성 장액맥락망막병증과 동반한 국소 맥락막 함몰 1예

목적: 만성 중심성 장액맥락망막병증과 동반된 국소 맥락막 함몰 1예를 보고하고자 한다. 증례요약: 48세 여자 환자가 20년 전부터 지속된 우안 시력저하를 주소로 내원하였다. 빛간섭단층촬영 검사상 우안의 국소적인 맥락막의 함몰이 관찰되었고, 신경망막층은 망막색소상피와 박리되어 있었다. 형광안저혈관조영술을 시행하였고, 초기 영상에서 국소 맥락막 함몰 부위의 과형광이 관찰되었고, 황반 주위로 다발성의 국소적인 과형광의 관찰과 함께 특징적으로 맥락막함몰 부위 직하방으로 위축된 망막색소상피로 인한 선상의 과형광이 관찰되었다. 두 가지 검사 소견을 종합해 볼 때, 만성 중심성 장액맥락망막병증에동반된 국소 맥락막 함몰을 진단할 수 있었고, 2개월이 지난 현재까지 악화 소견 없이 경과관찰 중이다.
Abstract
Purpose: To report a case of focal choroidal excavation associated with central serous chorioretinopathy. Case summary: A 48-year-old female presented with a 20-year history of visual disturbance. Focal choroidal excavation withneurosensory retinal detachment was detected in the right eye on optical coherence tomography. Fluorescein angiographyshowed hyperfluorescene in the area of excavation and multiple focal hyperfluorescences in the perimacular area. Vertically linearhyperfluorescene line was detected in the excavated area caused by retinal pigment epithelial atrophy. Based on the 2 diagnosticfindings, we diagnosed a focal choroidal excavation with central serous chorioretinopathy. No progression was detectedfor 2 months.
