불안정한 하악위를 가진 엇갈린 교합 환자에서 치료의치와 고정성 임플란트 보철을 이용한 전악 수복 증례

엇갈린 교합 환자는 수직고경의 붕괴 및 잔존 치아만을 이용한 저작 습관으로 인해 불안정한 교합이 발생한다. 하악 운동이 불안정한 경우, 안정된 운동 경로의 확립은 수복 치료의 장기적 예후를 확보하기 위한 핵심 요소이다. 본 증례의 환자는 엇갈린 교합으로 장기간 맞지 않는 국소의치를 사용하여 저작 패턴이 불안정해지고 습관적 하악 전돌 양상을 보였다. 환자의 기존 의치를 교합면이 평탄한 치료용 의치로 전환함으로써 하악 운동의 안정화를 유도할 수 있었다. 이후 안정화된 하악 위치를 기준으로 하여 무치악 부위에 임플란트를 식립하고, 고정성 보철물 및 임플란트 지지 원심 캔틸레버 고정성 보철물을 제작하여 수복하였다. 21개월의 경과 관찰 시 기계적, 생물학적 합병증은 관찰되지 않았고 기능적, 심미적 측면 모두 만족스러운 임상 결과를 보였기에 이를 보고하고자 한다.
Abstract
Patients who have a crossed occlusion with residual teeth frequently exhibit unstable occlusion due to collapsed vertical dimension and masticatory habits with remaining teeth. In a patient with unstable mandibular movements, establishing a stable mandibular movement is critical for ensuring the long-term stability of restorative treatment. In this case report, existing dentures of the patient with cross-arch unilateral occlusion were converted into treatment dentures with flat occlusion table for the stabilization of mandibular movements. Implants were inserted on the edentulous sites and fixed prostheses and implant-supported fixed prostheses were fabricated based on this position. The clinical outcomes were satisfactory in both functional performance and esthetic appearance.
