하악에 단일 치아만 남아있는 경우 전통적인 국소의치는 유지, 지지, 안정이 부족하고 치아에 과도한 응력이 집중되어 예후가 불량하다. 최근에는 국소의치 수복 시 소수의 임플란트를 전략적으로 식립하여 이러한 문제를 보완하는 치료 계획이 활용되고 있다. 특히 임플란트를 이용하여 서베이드 크라운 또는 고정성 국소의치를 제작할 경우 임플란트 어태치먼트를 이용하였을 때보다 장기적으로 더 좋은 예후를 보인다는 연구들이 보고되었다. 본 증례의 환자는 하악에 우측 견치만 남은 부분 무치악 환자로 양측 구치부에 임플란트를 식립한 후 서베이드 고정성 국소의치를 이용한 임플란트 보조 국소의치를 수복하여 임상적으로 만족스러운 결과를 얻었기에 이를 보고하는 바이다.
Abstract
With a solitary remaining tooth in the mandible, restoration with a conventional removable partial denture is often associated with insufficient retention, support, and stability, as well as excessive stress concentration on the remaining tooth, leading to an unfavorable prognosis. Recently, treatment plans involving the placement of a small number of strategically positioned implants to fabricate removable partial dentures have been adopted to overcome these limitations. The use of implant surveyed crowns is considered to offer superior long-term outcomes compared with implant attachments. This report describes the restoration of a partially edentulous mandible with a single remaining tooth using an implant-assisted removable partial denture assisted by a surveyed fixed partial denture following bilateral posterior implant placement, achieving favorable clinical results.
