코경유 눈물주머니코안연결술에서 폴리에테르 에스테르 우레탄의 임상적 효과

목적: 코경유 눈물주머니코안연결술 시 폴리에테르 에스테르 우레탄(NasoporeⓇ)을 내측 누공 부위에 채워넣은 후 임상효과를 알아보고자 하였다. 대상과 방법: 2010년 1월부터 2011년 2월까지 코눈물길막힘을 진단받고 코경유 눈물주머니코안연결술을 시행 받은 환자 77안을 무작위로 NasoporeⓇ를 내측 누공 부위에 채워넣은 군(41안)과 대조군으로 바세린 거즈를 채워넣은 군(36안)으로 나누어 전향적으로 조사하였다. 술 후 1, 2주, 1, 3, 6개월에 증상 호전 정도, 눈물소관 관류술, 비내시경 검사 결과 등을 평가하여 술 후 유착 및 내측 누공좁아짐의 정도를 알아보았다. 결과: 증상 호전 정도와 눈물소관 관류술에서는 두 군에서 유의한 차이는 없었다. 비내시경 검사에서 NasoporeⓇ를 사용한 군에서 술후 1, 2주, 1개월에 통계학적으로 유의하게 대조군보다 낮은 유착 및 내측 누공 좁아짐을 보였다(p＜0.05). 결론: 코경유 눈물주머니코안연결술 시 내측 누공 부위에 대한 NasoporeⓇ의 사용은 수술 후 초기에 유착을 감소시켜 임상적으로 좋은경과를 가져와 수술 보조 수단으로서 의미를 가진다.
Abstract
Purpose: To evaluate the clinical effect of intraoperative application of polyether ester urethane (Nasopore®) in endonasal dacryocystorhinostomy. Methods: Endonasal dacryocystorhinostomy was performed on 77 eyes diagnosed with nasolacrimal duct obstruction between January 2010 and February 2011. Nasopore® was applied to the newly-formed internal lacrimal opening in the experimental group (41 eyes), and vaseline gauze was applied in the control group (36 eyes). For the evaluation of clinical effects regarding postoperative adhesion and stenosis of the internal lacrimal opening, subjective symptom improvement,lacrimal irrigation tests, and endoscopic examinations were performed at 1 week, 2 weeks, 1 month, 3 months, and 6 months,postoperatively. Results: In subjective symptom improvement and lacrimal irrigation tests, differences between the 2 groups were not statistically significant. In endoscopic findings, the Nasopore® group showed significantly lower severity of adhesion and stenosis of the internal lacrimal opening at 1 week, 2 weeks, and 1 month after operation (p < 0.05). Conclusions: Application of the Nasopore® may effectively reduce initial postoperative adhesion or stenosis of the internal lacrimal opening after endonasal dacryocystorhinostomy and has usefulness as an auxiliary method.
