뇌성마비의 최신 지견

초록

=지난 수 십 년간 주산기학의 발전에도 불구하고 결국 뇌성마비의 발생 빈도는 변하지 않았다. 뇌성마비는 조산, 자궁내 감염, 유전적 소인, 환경적인 요인 등 많은 원인에 의한 다인성 질환으로 이해된다. 뇌성마비의 70-80%는 산전 원인에 기인하고 주산기 가사 (intrapartum asphyxia)는 뇌성마비의 원인의 약 10% 정도를 차지할 뿐이라는 많은 역학적 증거 및 연구 결과들에도 불구하고 뇌성마비가 의료진의 과실로 인하여 발생한 것처럼 오인되는 경우가 종종 발견된다. 따라서 뇌성마비에 관한 최신 지식을 숙지하고 있는 것은 산부인과 의사로서 중요한 일이라 하겠다. 이러한 배경 하에 저자는 최근 1년 동안 (2006 1월-2007 6월) PubMed를 바탕으로 뇌성마비에 관한 연구들을 검색, 그 중에서 뇌성마비의 재활의학과적 또는 정형외과적인 측면을 배제하고 최근 연구 결과들을 정리하고자 하였다. 또한 최근 연구 내용을 편의상 역학적 측면, 뇌성마비의 위험 인자 측면, 뇌성마비의 진단에 있어서 MRI의 유용성에 관한 내용 및 뇌성마비와 태반 병리와의 관련성에 관한 내용으로 분류하였다.
