허혈성 대장염과 동반된 대장 기종 2예

초록

대장 기종(pneumatosis coli)은 위장관의 장막 또는 점막하층에 다발성 낭포를 생성하는 드문 질환이다. 85%에서 다른 소화기, 호흡기 혹은 교원성 질환을 동반하게 된다. 이 질환만의 특이 증상은 없으며, 혈변, 복통, 설사 또는 변비 등의 비특이적 소화기 증상이나 동반 질환에 의한 증상을 보인다. 대장내시경 검사 및 전산화 단층 촬영으로 진단 할 수 있으며, 치료는 동반 질환에 대한 치료와 산소 흡입, 항생제 투여, 수술 등으로 치료 할 수 있다. 대체로 양호한 임상 경과와 예후를 지니나, 약 3%에서 장폐쇄, 장염전, 장중첩증, 장출혈, 장천공 등의 합병증이 발생할 수 있다. 저자들은 혈변을 주소로 내원하여 시행한 대장내시경 검사 상에서 국내 최초로 허혈성 대장염과 동반한 대장 기종을 발견하였으며, metronidazole과 저농도의 산소흡입요법의 보존적 방법으로 치료하였기에 문헌고찰과 함께 보고한다.
