베체트병은 만성적, 재발성의 전신 혈관염으로 재발성 구강 및 성기 궤양, 피부 병변, 눈 병변이 특징적이다. 다른 자기 면역 질환과 달리 베체트병에서 위 림프종은 드물게 보고된다. 40세 남자로 4년 전부터 베체트병으로 치료 도중에 내원 1달 전에 발생한 상복부 통증을 주소로 내원하여 상부 위장관 내시경에서 큰 궤양성 종괴가 보여 시행한 조직 검사에서 미만성 거대 B 세포 림프종으로 진단하였고 rituximab-CHOP의 항암 치료 후 현재 3년 동안 완전 관해를 유지하여 외래 경과 관찰 중이다. 저자는 회맹판을 침범한 베체트병으로 치료 중 발생한 위의 미만성 거대 B 세포 림프종 1예를 경험하여 문헌 고찰과 함께 보고한다.
Abstract
Behçet’s disease is chronic, relapsing, multi-systemic inflammatory vasculitis. Unlikely the other autoimmune diseases, gastric non-Hodgkin’s lymphoma has rarely been reported in a patient with Behçet’s disease. We report here on a 40-year-old man who had Behçet’s disease for several years and who suffered recurrent oral ulcer, genital ulcer and skin lesion. The patient was treated with azathioprine and intermittent oral steroid. He complained of epigastric pain for 1 month, and he had a large ulcerative lesion seen on endoscopic evaluation. We confirmed the lesion was a gastric, diffuse, large B cell lymphoma by endoscopic biopsy. The patient was treated with the rituximab-CHOP regimen, and he has achieved a complete remission for 3 years. We report here on a case of gastric non-Hodgkin’s lymphoma in a patient with Behçet’s disease and who also had an ileocecal valve ulcer.
